Director of Care Coordination

Our partner is looking for a Director of Care Coordination in Akron, OH.

The Director of Care Coordination (DCC) is responsible for the systems and licensed and non-licensed staff that ensure the patient and their support people receive the care they need from providers and resources within the medical neighborhood in an equitable way to ensure an outstanding patient experience and promote positive health outcomes. Systems must be designed to address health equity both within and outside of health care including the social, financial, communication, medical, psychological and logistical barriers to accessing health care.

• Understanding the organization's throughput and flow objectives, barriers, and opportunities, and formulating the vision, strategy, and direction for the Care Coordination model.
• Leads and supports the organization's pursuit for quality improvement through NCQA accreditation and other means.
• The Director of Care Coordination will determine how to operationalize this strategy, managing, motivating, and directing the staff and leadership in the support of high-quality patient care, safety, financial improvements, and optimal patient flow.
• The Director collaborates with internal and external service leaders to ensure optimal care coordination and patient flow to needed levels of care to ensure best practice and patient outcomes.
• Fosters, develops, coordinates, and maintains relations and networks across the service area in collaboration with the Referral Resource Manager to support patients with their abortion care, gender affirming care, gynecologic care, smoking cessation, behavioral health, primary care and mental health needs.
• Designs care coordination service delivery aligned with organizational goals and clinical, financial, and experience outcomes across all service lines, collaborating with multidisciplinary teams to execute care management responsibilities including a close partnership with clinicians, operations, and analytics functions.
• Provides leadership and consultations in all aspects of care coordination, as well as develops, implements, directs and evaluates an operational plan for care coordination that aligns with the organizational strategic plan to ensure that processes meet or exceed regulatory and industry standards.
• Develops measures of success around the care coordination program to include the impact on patient health outcomes, patient retention, overall patient satisfaction and willingness to recommend PPGOH for care.
• Reports and analyzes the impact of the program as related to the Social Determinants of Health.
• Serves as a highly visible leader, colleague, and mentor, providing leadership, direction, and education.
• Collaborates with external partners, affiliate leaders and physician advisors in the development and execution of the care coordination program.
• Centralized Follow-up Supervisor, Clinical Quality Manager(s), Referral Resource Manager, Patient Navigator(s)

Requirements

• LISW or APRN preferred.
• Bachelor's Degree in Nursing, Social Work, Health Care Administration, Public Health, or other related field.
• 5 years supervisory or program management and improvement experience. Additional experience will be considered in lieu of a degree.
• Previous experience overseeing licensed staff and various functions of health care delivery.
• Previous experience managing and/or building referral networks with an emphasis on health equity.
• Customer service or patient care experience required.
• Previous experience working with individuals of a diverse socio-economic background.
• Exceptional interpersonal, team-building, and external relationship building skills to establish and maintain strong relationships with a diverse group of stakeholders.
• High level of comfort in dynamic, shifting environments.
• Strong organizational and communication skills
• Must be able to work independently with the ability to prioritize and lead with minimal supervision
• Must maintain confidentiality
