Who Are We?

Concerto Renal Services is an industry leader in nursing home dialysis. We are a rapidly expanding business with a simple mission: reimagining dialysis care, one community at a time. Our model focuses on ensuring those with end-stage renal disease receive the best possible care through a more convenient, centralized, continuous care model.

Why is this Role Essential?

Concerto is seeking Regional Support Nurses (Dialysis Nurse Float) to join its team! This is a salaried float nurse role responsible for providing RN coverage across assigned markets. Regional Support Nurses are not assigned to one facility and are expected to travel across their assigned area of responsibility as needed to cover vacancies, leaves of absence, or emergency staffing needs. Regional Support Nurses supervise dialysis care in accordance with policy, procedure, and training, and in compliance with state and federal regulations. They are responsible for unit operations during coverage, working collaboratively with members of the hemodialysis healthcare team while assuming a “hands-on” role in patient care.

What Will You Do?

  • Provide excellent customer service to all external and internal stakeholders including patients/families, staff, physicians, and long-term care staff.
  • Deliver continuous education to patients on renal disease, vascular access, dialysis therapy, and related health concerns.
  • Delegate tasks to dialysis technicians to ensure safe and effective dialysis delivery as prescribed.
  • Supervise and direct dialysis technicians in accordance with compliance standards, policies, procedures, and federal/state regulations.
  • Provide, monitor, and supervise hemodialysis care per Concerto’s established policies.
  • Conduct pre- and post-treatment assessments, document interventions, and evaluate patient response.
  • Ensure accuracy and timeliness of documentation completed by dialysis technicians.
  • Collect and package lab specimens in compliance with Department of Transportation (DOT) policies.
  • Collaborate with physicians and healthcare team members to interpret, adjust, and coordinate care.
  • Ensure all relevant data (physician orders, labs, medications, treatment parameters, patient condition, etc.) is documented accurately in the EMR.
  • Review lab results promptly and report critical values to physicians.
  • Complete patient assessments and care plans on time.
  • Administer medications as prescribed, with appropriate documentation.
  • Maintain safe, efficient, and effective operations during each assigned shift.
