Position: RN Visit Nurse for Home Health

RN Wage Range:  $85,000-$87,000 annually, depending on experience (1.0 FTE) *salary will be pro-rated accordingly for the 0.9 positions

Service Area: South Central (coverage area: St. Louis Park, Richfield, Bloomington, Burnsville, Lakeville & surrounding). Plus, mileage reimbursement of .70 cents per mile.

Schedule: There are 3 FTE schedule options to choose from, each schedule options include a holiday rotation (2 holidays per year)

Option 1: 0.9 FTE *salary will be pro-rated accordingly to reflect 0.9 FTE

Every Friday - Sunday: (3) three 12 hour shifts, 8am-8pm

Option 2: 0.9 FTE *salary will be pro-rated accordingly to reflect 0.9 FTE

Every Saturday - Monday: (3) three 12 hour shifts, 8am-8pm

Option 3: 1.0 FTE

Every Sunday, Monday, Wednesday, Saturday: (4) four 10 hour shifts 8am-6pm

The RN Visit Nurse is responsible to plan, organize and direct home care services and is experienced in nursing, with emphasis on community health education/experience.  The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.    

 

Position Functions & Responsibilities:

  • Completes an initial assessment of client and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
  • Regularly re-evaluates client nursing needs.
  • Initiates the plan of care and makes necessary revisions as client status and needs change.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the client and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.  Provides direct client care as defined in the State Nurse Practice Act.
  • Counsels the client and family in meeting nursing and related needs.
  • Provides health care instructions to the client as appropriate per assessment and plan of care.
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the client.
  • Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
  • Lead by example in establishing rapport with clients, clinicians, partners, and the community.
