Hospice Registered Nurse (RN), Full Time, On-Call Weekends Only

Providence at Home is a growing division of Providence Life Services, which provides a continuum of healthcare, focused on senior living. Providence Hospice is seeking a RN Hospice Case Manager, Full Time in St. John, IN. This position will require the RN to work Friday - Monday, 4:30PM - 8:00AM. On-Call on Weekends only.

Summary of essential job functions:

• Provide skilled nursing care
• Provide case management services
• Conduct initial assessments of new patients
• Travel to patients wherever they reside: Home, nursing facility etc.

Requirements

• Active Registered Nurse (RN) License in the state of Indiana.
• At least 1 year of RN experience, preferably in Hospice or Home Health.
• Drivers License in good standing.
