Central Intake and Administrative Coordinator

Job Type:  Fulltime, Monday - Friday

Location:  Vestal, NY

Wage:  $21.63 - $26.44


DUTIES AND RESPONSIBILITIES: The Central Intake Coordinator oversees and manages internal and external outreach efforts, communications and related activities which effectively convey the supports and services offered by ACHIEVE. The Central Intake Coordinator supports the Agency’s commitment to supporting opportunities for people to achieve their life goals at home, school, work and in the community by connecting people to ACHIEVE’s services. This position’s specific duties and responsibilities include, but are not limited to the subsequent items, and are characterized in the following five standards of performance, defined as below:


I.    JOB KNOWLEDGE

  • Cultivates relationships with new customers and referring organizations.
  • Promotes Agency services, development initiatives and opportunities.
  • Educates new customers and referring organizations about agency services.
  • Maintains open communication with Agency divisions and departments to ensure      continuity and efficiency of service delivery. 
  • Coordinates with Agency program divisions to track program vacancies.
  • Organizes and screens referrals for all programs
  • Coordinates with Agency program support divisions to provide input on referrals.
  • Gather and submit required documentation for program intake. 
  • Assists program leadership with the development of goals and IPOP. 
  • Participates in Intake and Life Plan meetings as needed to discuss additional services a person may want. 
  • Complete all requested and normal clerical responsibilities for the Chief Executive Officer and Chief Operating Officer.
  • Attend requested meetings, events, and Board of Director Meetings; prepare/transmit required or requested documents for such meetings/events.
  • Act as role model to people receiving services and staff 
  • Any additional duties as assigned


Requirements

SPECIAL REQUIREMENTS:

Possess good interpersonal, written, oral, and communication abilities and effective time management, organizational and problem-solving skills 


Able to work scheduled hours that may include mandatory overtime, weekend, and/or holiday timeframes. Also maintain an acceptable work attendance as defined in the agency’s personnel policy manual and handbook.


Able to work independently and as a member of a team


Able to manage multiple projects and complete assignments with extreme attention to detail; grammar, spelling, punctuation, and report/letter writing must be excellent


Be computer literate with knowledge in agency operating systems and applicable software required for department needs.


Demonstrate sound judgment in normal work and emergency situations


Must physically be able to perform required certifications and lift 50 pounds 


Possess a valid driver’s license acceptable to the agency’s insurance carrier


QUALIFICATIONS:

Bachelor’s Degree in Social Work, or a related Human Service field

2-4 years of relevant experience with at least 2 years of experience working with individuals with developmental disabilities.

A combination of education and experience that in the opinion of the reviewing agency, exceeds the above standards of the program
