Care Team Technical Writer / Patient Resource SpecialistPosition Summary: The Care Team Technical Writer / Patient Resource Specialist is a hybrid administrative-clinical support role responsible for developing compliant documentation, supporting patient resources, and coordinating critical services. This role is central to ensuring patients receive timely, accurate care and access to needed benefits, medications, equipment, and travel resources. The individual will work closely with nurses, Patient Coordinators, physicians, and home health aides to ensure that care plans and supportive services align with Department of Labor (DOL) standards and organizational values.

Key Responsibilities
- Technical Writing & Documentation:
- Draft and finalize letters of medical necessity for home care, durable medical equipment (DME), medications, therapies, and other services requiring DOL approval.
- Prepare and process initial and renewal documentation for home health services every six months.
- Maintain accurate case management charting, care plans, and documentation logs.
- Represent the organization in written communications with providers, ensuring all documents meet compliance standards.
- Request, review, and file incoming medical records as part of the documentation process.
- Patient Resource Support:
- Assist patients in navigating DOL reimbursement paperwork for travel, lodging, meals, and other medically necessary expenses.
- Support patients with pharmacy needs, including coordinating prior authorizations and enrolling them in medication assistance programs.
- Help identify and connect patients to relevant community resources based on social, financial, or medical needs.
- Maintain a resource database for use by care teams and patients.
- Team Collaboration & Communication:
- Partner with Patient Coordinators to track patient needs and ensure timely resolution of reimbursement and resource issues.
- Communicate with physicians and service providers to obtain supporting documentation and coordinate care delivery.
- Provide education to patients, caregivers, and team members on required paperwork, care coordination, and services.
- Collaborate with interdisciplinary care teams and manage a caseload of 150–200 patients.
- Administrative Support:
- Accurately log and file documentation related to authorizations, incident reports, and care coordination.
- Cover appointments or perform in-home patient visits when needed to support recertification efforts or relationship-building.
- Travel locally up to 5% of the time to support care coordination and patient satisfaction.
Qualifications:
- Qualifications:
- 1–3 years of experience in Care Coordination, Medical Records, Billing, or Revenue Cycle strongly preferred.
- Experience in home health or healthcare setting preferred.
- Pre-authorization experience highly desired.
- Proficient in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint).
- Strong written and verbal communication skills.
- High attention to detail and strong organizational skills.
- Ability to manage multiple priorities and meet deadlines.
- Comfortable working independently with minimal supervision.
- Must pass a Department of Health background check.
- Valid driver’s license and reliable transportation required.
