The Patient Navigator will be responsible for:
- Working collaboratively with the Endocrine Mental Health Team in building a new model of care to support the psychosocial needs of patients and families living with a chronic medical condition
- Meeting with/calling patients and families to review the implementation of care plans. Referring patients to services frequently used within the Endocrinology clinic and specialized services as needed. Explaining hospital/community based resources to patients and families, providing documentation as needed
- Assessing patients for referrals to resources. Acting as a liaison with internal and community resources. May contact hospital and community resources to ensure access to services
- May assist patients and families with scheduling multidisciplinary appointments. Reviewing appointment schedule and appointment instructions. Coordinating logistics for plan adherence such as appointment reminders and arranging transportation
- Assisting patients with applying for insurance. Providing insurance information to care teams to determine eligibility for services
- Maintaining relationships for accessing community based resources. Providing outreach and maintaining positive working relationships with providers/administrative staff, including physicians, office staff, nurses and social services staff. Reporting outcomes of referrals to referral source
- Documenting patient care and collecting required data. Tracking outcomes and supporting strategic planning processes. Developing and maintaining data
- Attending and participating in care planning meetings to prepare patients for discharge or continuing care
- High school diploma/GED required; Bachelor’s degree in Psychology, Social Work, or Mental Health field preferred
- Minimum of one year of closely-related experience required. Experience with the clinic population or work with community resources is preferred
- Ability to communicate effectively both orally and in writing and provide empathy in difficult interpersonal situations
- Work requires the analytical skills to resolve problems that require the use of basic scientific, mathematical or technical principles
- Bilingual (Spanish) a plus
