This position involves up to 75% travel in Sangamon County, IL and surrounding areas.

The schedule for this position is Monday-Friday 8a-5p CST.

Position Summary
Coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall
wellness. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as
opportunities to enhance a member’s overall wellness through integration. Services strategies policies and programs are comprised of
network management and clinical coverage policies.
This is an Exempt position.

Required Qualifications

• Minimum 2+ years of clinical experience required
• Active and unrestricted RN license in the state of IL
• Willing and able to travel 50-75% of their time using their own vehicle to meet with members face to face in Sangamon County, IL and surrounding areas. Valid drivers license required. Reliable transportation required. Mileage is reimbursed per our company expense reimbursement policy

Preferred Qualifications

• Case management and discharge planning experience
• Managed Care experience
• Microsoft Office Suites experience

Education

• Bachelors degree preferred, minimum of Associates degree required

Pay Range

The typical pay range for this role is:

$66,575.00 - $142,576.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
