Intake Transfer RN Nights - 3/4 Split

The Transfer Center RN is responsible for evaluating, coordinating, processing, screening, and documentation of patient transfers into the SABHH. The Transfer Center RN will collaborate with relevant business partners for appropriate level of care. The Transfer Center RN will be responsible for obtaining appropriate medical and demographic information and coordinating communication between the referring and receiving physician as well as between healthcare facilities. The Transfer Center RN will efficiently engage appropriate departments in the transfer process. The Transfer Center RN will utilize problem solving and customer service skills to determine the best course of action for the patient, the physician, and the hospital by working closely with facility House Supervisors, referring physicians, ED, and inpatient staff to ensure the effective and efficient transfer of every patient.
Essential Duties:
- Review collateral information on each patient.
- Address medical concerns indicated if the patient has DX for HTA, HIV, UTI, etc.
- Make sure we have valid doctor’s orders.
- Detox: If the patient endorses EtOH detox, benzo detox, or heroin detox, ensure we have doctor’s orders for CIWA/COWS
- Send collateral, including ED, at the time of acceptance. Must have ED when accepted.
- If the patient is psychotic and unable to sign in, we must have ED or OPC.
- If the patient requires ambulatory devices (wheelchair, cane, brace) obtain doctor’s orders.
- Enter all patient information into AIS.
- Use appropriate phone etiquette for quality calls and address all customer questions and concerns.
- Send the nursing report for MOT patients.
- Verify insurance, send collateral, insurance information to intake along with the nursing report.
- Communicate with the intake supervisor and admissions coordinator on available beds & incoming MOTs
- Provide an update to the Director of Admissions and the Assistant Director of Admissions on how many patients we are accepting at the end of each shift and how many patients need to be followed up with.
- At shift change, communicate how many patients need to be followed up with to the oncoming Transfer RN.
- Upload every fax in AIS with correct information and insurance regardless if we deflect the patient.
- Mobile assessment visits are completed in a timely manner, and in accordance to hospital policy
- Administrative approval obtained for all admissions
- Safety problems are identified and corrective actions taken
- Answer telephone calls to take messages, answer questions, and provide information during non- business hours or when switchboard is closed.
- Responds to and documents telephone and face-to-face inquiries from persons seeking mental health care or information about mental health-related services.
- Ensures each patient's unique needs are identified and incorporated into a comprehensive, individualized care plan including the severity of the disease, condition, impairment/disability as important elements of the plan.
- Complies with laws, rules, regulations, protocols, and procedures.
- Maintains patient confidentiality.
- Stays abreast of organization's programs and services.

Requirements
Job Requirements
Must be 21 years old or older
Education - RN, ADN or BSN from an accredited college or university.
Experience – Minimum of 2 years clinical experience, preferably in a behavioral healthcare setting, phone triage, tele-health or case management or combination of education and experience. Basic teaching and training skills a plus.
Required Licenses - Valid Texas RN license in good standing required. CPR (must be obtained within 30 days upon hire) BLS, ACLS, CPI certifications a plus.
Additional Requirements: Must pass a medication administration exam before administering medications. Must possess or obtain a valid CPR certification and SAMA training within 30 days of hire.
Knowledge Skills and Abilities
· Ability to communicate effectively both orally and in writing i.e. legible handwriting and ability to read and understand written instructions to include writing techniques for reports and appeals and verbal presentations for telephone interactions.
· Ability to apply professional nursing care principles and procedures in the evaluation and treatment of psychiatric patients. Develop interpersonal therapeutic relationships with psychiatric patients.
