Job Description
As a Clinical Pharmacy Specialist, you will provide high-level direct patient care for a specialized population, ensuring adherence to professional standards. You will advise and consult with patients, families, and medical staff. Your role includes participating in research and drug use/outcome management programs, publishing data, and presenting at meetings. Additionally, you will instruct and train pharmacy residents and students, oversee clinical services provided by pharmacists, and mentor clinical pharmacists.
Clinical Pharmacy Services
- Participate in patient care with medical or surgical teams.
- Assist with therapeutic decisions, including drug selection, dosing, and monitoring.
- Develop patient care plans with the medical team.
- Recommend therapies based on medical literature and clinical experience.
- Provide clear, succinct recommendations.
- Consider age, weight, and organ function in evaluations.
- Interview patients and conduct physical assessments.
- Interpret lab data to develop and monitor treatment plans.
- Apply pharmacokinetic principles for safe, effective regimens.
- Respond to therapeutic queries from healthcare providers.
- Identify and correct drug-related problems.
- Establish procedures to detect and avoid drug interactions.
- Perform basic cardiac life support and manage drug therapy during emergencies.
- Ensure continuity of care by collaborating with other health professionals.
- Counsel patients and caregivers on medication administration and potential adverse reactions.
Drug Use, Outcome and Formulary Management
- Ensure adherence to clinical protocols and guidelines among multidisciplinary care providers.
- Collaborate with the Coordinator of Drug Use and Outcome Management to enhance drug therapy outcomes and patient care quality.
- Implement and oversee drug use management programs with the Coordinator of Drug Use and Outcome Management and Formulary Management.
- Design drug use criteria, collect patient data, and provide analysis and reports for the Medication Use Evaluation Program.
- Assist in developing clinical pathways and managed care initiatives.
- Contribute to formulary management by developing drug monographs, treatment protocols, policies, algorithms, and guidelines.
- Document pharmaceutical care interventions as part of the departmental documentation program.
Qualifications
Education:
- Required: Bachelor of Pharmacy from an accredited college or university (American Association of Colleges of Pharmacy).
- Preferred: Master of Science, PhD, or Doctor of Pharmacy from an accredited institution.
