Under limited supervision, performs distributive and clinical pharmacy functions in accordance with regulatory requirements and organizational quality initiatives. Distributive functions include: performing the final check of appropriate medications prior to dispensing; maintaining the required security and control of medications; assuring the completion of appropriate documents and records; supervising pharmacy support staff. Clinical functions include: medication order review and computer entry; providing consultative drug information and recommendations to physicians and nurses; conducting patient assessments and making appropriate interventions; administering established clinical programs and medication management programs. This position must be able to demonstrate the knowledge and skills necessary to provide care and service appropriate to the age of the patients served in their area.Pay Range: $119,225.60 - 178,838.40 USD Annual
Employment Type: 80 hours every 2 weeks.
Schedule: Monday-Friday 6:30am-3:30pm; Weekend/Holiday rotation; Occasional Mid-shift & 2nd shift 9am to 5:30pm; 1:30pm-10:00pm.
Requirements: Doctor of Pharmacy (PharmD) and Licensed to practice in the State of Michigan.
Performs medication order interpretation, evaluation, and processing in accordance with organizational expectations
Accurately interprets and evaluates medication orders as evidenced by lack of physician /peer complaints or occurrences.
Monitors drug therapy regimens for contraindications, drug-drug interactions, allergies, therapeutic duplications, and appropriateness of drug/dose; addresses discrepancies in accordance with departmental expectations.
Applies pertinent medication management programs (High risk medications, therapeutic interchanges, restricted dose limits, clinical dosing, etc.) at the time of interpreting/evaluating orders as evidenced by lack of physician / peer complaints or occurrences.
Appropriately communicates with physicians and nurses to resolve medication related problems, clarify questionable orders, and provide medication therapy recommendations.
Provides and documents appropriate modifications to medication orders prior to preparation and dispensing.
Transcribes medication orders into electronic medication record profiles accurately and in a timely fashion as evidenced by lack of nursing/peer complaints or occurrences.
Oversees medication preparation, distribution, security, and management in accordance with regulatory requirements and organization expectations.
Maintains responsibility for the accuracy and timeliness of daily medication (sterile and non-sterile) preparation practices as evidenced by countersignatures and lack of peer complaints or occurrences.
Maintains daily oversight responsibility of the accuracy and timeliness of medication distribution processes.
Maintains oversight accountability for security, control, distribution, and documentation of medications (including controls).
Assures complete documentation of medication preparation, distribution and medication security documentation as evidences by logs and co-signatures.
Performs clinical pharmacy functions and administers established clinical programs.
Performs, accurately and completely, established clinical medication protocols and initiatives (demerol restrictions, clozaril and warfarin use oversight, antibiotic reviews, core measure follow-up, etc.)in accordance with departmental expectation.
Administers accurately and completely, medication dosing programs (including renal, heparin, aminoglycoside and vancomycin, pneumonia antibiotics) as evidenced by clinical documentation.
Provides clinical medication consultation, clarification, and education to physicians, nurses, and patients.
Processes and evaluates Adverse drug reaction reports and follow up in an accurate and timely fashion as evidenced by documentation.
Participates in patient care rounds and patient specific chart reviews.
Assesses patient status and medication regimen in order to make recommendations which maximize the value of drug therapy and achieve desired patient outcomes.
Sustains the formulary by mimicking non-formulary medication procurements utilizing therapeutic substitution protocols and promoting rational drug therapy selection.
Completes required documentation of clinical activities and interventions in accordance with departmental practices.
