Your Specific Responsibilities:
This is a highly responsible position reporting to the Community Health Management Programs Administrator. The goal of this position and its workplan is to help residents and community partners assess and address the Social Determinants of Health (SDOH) that influence the county’s health disparities related to COVID-19 and other diseases and disorders.
The position is responsible for creation/implementation/maintenance of the Health Equity Team, Health Equity Task Force, County Health Equity Coalition and the County Health Equity Plan, related projects and programs, and programmatic data collection and reporting.
• Maintain a CHD Health Equity Team consisting of staff from various programs within the County Health Department (CHD) to strengthen the internal infrastructure of the CHD and its ability to support health equity efforts. Lead the CHD Health Equity Team in hosting a promotional event in the month of April to raise awareness of National Minority Health Month and all other observances as requested.
• Maintain a Health Equity Plan Task Force including the Health Equity Team and partners to develop and update the County Health Equity Plan. Ensure Health Equity Task Force membership represents organizations with the capacity and focus to implement programs and projects to address the social determents of health and health equity.
• Maintain a County Health Equity Coalition that includes stakeholders who are interested in or involved in the progress of health equity. Identify and establish collaborations with critical Health Equity Coalition members affiliated with populations at higher risk and that are underserved for COVID-19 and other diseases and disorders.
• Maintain/update the County Health Equity Plan using relevant partners to develop projects and program changes to mitigate the effects of SDOH in communities experiencing health disparities.
The Health Equity Plan should include and utilize:
• Specific activities to address the social determinants of health (SDOH) to improve health outcomes of racial/ethnic (R/E) minority and other vulnerable populations.
• Specific activities to develop a shared understanding of HE in the county through training and dissemination of resources.
• Specific activities to increase coordination and collaboration among cross-sector partners.
• Specific activities to increase awareness of the local HE Plan.
• The He Plan should use:
• Data to identify sub-county geographical areas (census tracts, zip codes and or neighborhoods) to prioritize activities.
• Evidence-based and promising approaches to inform activity planning.
• A Health in All Policies approach to advance HE.
Develop and maintain a comprehensive distribution list of county partners to expand reach of health equity information.
• Maintain a comprehensive list of partners serving R/E minority populations and other vulnerable populations and addressing the SDOH. Consider including community- and faith-based organizations, non-profits, colleges and universities, children services councils, county school boards, health services councils, social service providers, beauty and barber shops, local government, chambers of commerce, migrant farmworker coalitions, etc.
• Regularly disseminate information to the appropriate partners regarding training, resources, events, grant opportunities, etc.
• Maintain documentation of disseminated resources.
• Identify and coordinate with county-level maternal and infant, HIV/AIDS, chronic disease and health promotion programs, etc., as needed to support the implementation of the HE Plan.
Meetings: Attend the monthly statewide COVID Health Disparities virtual meetings, monthly Minority Health and Health Equity Liaison meetings as requested and required.
Reporting: Submit the HE Plan Project management tool, quarterly project reports and all other required documentation in an accurate and timely manner.
Budget: Work with the Community Health Management Programs Administrator in budget development and monitoring. Submit the budget summary and budget reports in an accurate and timely manner.
