Role Description
The Manager of Population Health will immediately become an important contributor to the Oak Street Health team. S/he will execute a set of responsibilities to enable the success of our clinical model, which is core to Oak Street's success.
Responsibilities
Specific responsibilities include:
- Key focus on health outcomes and controlling medical costs in our patient population
- Owning the strategy, process, and implementation of key operational roll-outs (will involve on-the-ground support at our clinics)
- Leading the clinical development of new care programs, including coordinating and synthesizing feedback from care teams and reviewing evidence-based guidelines
- Reporting and analytics in support of our Care Teams (physicians, nurses, care managers)
- Establishing and maintaining relationships with both internal and external stakeholders
- Other duties, as assigned
The role reports to the Director of Population Health Strategy & Analytics and has significant
opportunity for rapid advancement.
