SOUND Behavioral Health is hiring a Transition Support Program (TSP) Clinician at our Capitol Hill clinic in Seattle, WA! 

 

The Transition Support Program (TSP) is a community-based outreach team that works with clients in the first 90 days after involuntary psychiatric hospitalizations to assist in making connections to outpatient care.  TSP Clinicians work intensively with a small caseload of clients, with a supportive team.

 

TSP clinicians become true resource experts as they work with clients using harm reduction strategies, motivational interviewing, and solution-focused interventions to move toward positive care transitions that reduce repeat hospitalizations.

 

Building on the mantra of “Teach, don’t take over,” TSP Clinicians provide supportive contact with clients while they are hospitalized - helping clients develop their own goals and assist in taking achievable steps toward meeting those goals.

 

TSP Clinicians promote client self-advocacy and engagement in community services by providing education, coaching, and resources to promote ongoing self-management of clients’ medical and behavioral health conditions.

Responsibilities

ESSENTIAL FUNCTIONS AND DUTIES 

  • Provides direct services and/or internal and external organization referrals for clients. 
  • Develops treatment plans and maintains accurate and timely clinical records according to organization policy. 
  • Prepares necessary reports, summaries, and progress notes and incorporate them into clinical records. 
  • Performs consultations and liaison duties with other agencies and resources on behalf of the client. 
  • Provides outreach, engagement, care coordination, and intervention to clients while in the hospital and post-discharge in their communities. 
  • Assists clients to remain stable in the community, thereby reducing the use of hospitals or involuntary treatment services. 
  • Monitors medication regimes and treatment compliance. 
  • Monitors utilization of services and the success of outcome measures. 
  • Participates in team member meetings, consultations, case presentations, and in-service training. 
  • Participates in ongoing program development and assessment. 
  • Performs other duties as assigned.

Establishes and maintains positive, ethical, and professional working relationships with clinical team members, clients, and outside organizations. Relates to a wide variety of people, including members of ethnic and sexual minority populations, and is respectful of individual and cultural differences. 

 

Qualifications

EDUCATION AND EXPERIENCE 

  • A Bachelor's degree in social work, psychology, or a counseling-related field, with a strong emphasis on working with families and community agencies. 
  • A minimum of one year of clinical experience as a clinician or case manager is preferred. 
  • Demonstrated skills in clinical assessment and treatment plans (preferred. 

 

KNOWLEDGE, ABILITIES, AND SKILLS 

 

  • Assertiveness and interest in creating strong therapeutic relationships with clients in challenging circumstances 
  • Inclusiveness and compassion balanced with great boundaries and a strong emphasis on personal safety. 
  • Passion for working with diverse, underserved populations and interest in learning about the realities of homelessness and mental health. 
  • Strong clinical judgment and a strengths-based world view. 
  • Understands the principles of individual therapy, group therapy, and/or case management approach to providing services. 
  • Possesses strong working knowledge of the mental and/or behavioral health and support resources of King County and is familiar with state and local health delivery systems, including knowledge of various funding principles and clinical services mandates. 
  • Understands recovery concepts in behavioral health; synthesizes and applies, as appropriate to their work at Sound. 
  • Ability to learn new tasks and skills quickly and to manage and prioritize multiple tasks while simultaneously managing crises. 
  • Able to demonstrate the capacity to appreciate and work well with a complex and diverse population. 
  • Familiar with the APIC and Coleman care coordination models; 
  • Understand and have experience with motivational interviewing; 

 

CERTIFICATES, LICENSES, & REGISTRATIONS 

  • Must be able to obtain a WA State Agency Affiliated Counselor credential within the first 60 days of hire. 
  • Washington State drive
