WHAT YOU CAN EXPECT

  • Participates in people management activities for direct team members such as conducting performance evaluations, disciplinary actions, and interviews.

  • Implements interventions based on risk stratification, identified quality metric and patient centered goals utilizing multiple tools to accomplish.

  • Participate in performance improvement activities and team coaching.

  • Provides education to the healthcare team regarding disease management and evidence-based practice guidelines with appropriate tools and intervenes as appropriate with patients and Interdisciplinary Care Team.

  • Guides documentation standards and communications for patient’s transitions of care.

  • Maintains a seamless and coordinated acute/post-acute care continuum for ACO patients discharged from Franciscan Health facilities systemwide.

  • Support through management oversight appropriate interventions and document to meet the needs of the patient population being managed by the Care Team. Utilizes best practices and standards based on scientific principles, theoretical knowledge, and clinical expertise.

  • Utilizes and promotes motivational and health-coaching techniques to engage the Care Team.

  • Oversees Care Management team to assure reassessment and modification of the care plan as necessary to achieve health goals.

  • Understand and apply federal and state regulations effectively. Demonstrate knowledge of ACO/ICM organizational policies.

  • Supports program data collection and analysis for PAC CCNs systemwide. Facilitates the creation and follow through on action plans that address deficiencies.

  • Facilitates root cause analyses for returns to the emergency department, re-admissions, etc.

QUALIFICATIONS

  • Associate's Degree in Nursing - Required

  • Bachelor's Degree in Nursing - Preferred

  • 5 years Hospital/Ambulatory experience - Required

  • 5 years Current nursing experience - Required

  • 5 years Case management/coordination experience - Required

  • Prior leadership experience (i.e. supervisor, manager) - Preferred

  • Previous experience with hospital information systems (i.e. order entry, results reporting) - Preferred

  • Registered Nurse (RN) - in state of practice - Required

  • Basic Life Support Program (BLS) - American Heart Association - Required

  • Certified Case Manager (CCM) - Preferred

  • Driver's License - Required

  • Auto Insurance - Required

TRAVEL IS REQUIRED:
