A Brief Overview:
This position floats to all inpatient units to provides counseling and discharge planning services to patients who in need of ongoing behavioral health and/or medical care following discharge due to changing census and acuity level. Coordinating discharge planning at the direction of the patient's treatment team. Obtaining appropriate releases, communicating with other care providers and collaborating with patients, families and the treatment team to develop, arrange and document a comprehensive discharge plan.

What you will do:

  • Demonstrates the knowledge and skills necessary to provide care appropriate to the age of patients served. This includes knowledge of the physical, social and psychological needs of patients served and the ability to respond appropriately to those needs.
  • Floats to all inpatient units and demonstrates ability to quickly shift between clinical responsibilities and discharge planning.
  • Conducts psychosocial assessments, family sessions, group and carries a therapy caseload as needed.
  • Demonstrates a comprehensive knowledge of community resources (12 step support meetings, recovery home and residential treatment facilities) and medical allied staff expertise to assure an appropriate discharge liaison is implemented.
  • Provides educational support through individual and group sessions focusing on orientation, relapse prevention and discharge planning.
  • Attends treatment team meeting and care conferences to discuss key social, financial, and discharge planning factors which may affect patients' responses to treatment is necessary.
  • Educates patients and patient families regarding the effects of illness and disability on personal adjustment, family dynamics, relationships, social issues and the like.
  • Maintains accurate and timely medical records including psycho-social assessments, family session and daily process notes to include school personnel and other community collateral involvement. Provides descriptions of therapeutic interventions and patient response for discharge planning process including after visit summary.
  • Continuously communicates discharge plan to patients, patient families and staff in order to help patient with adjustment to hospitalization and to facilitate discharge plan.
  • Meets with patients to identify patient goals for discharge, available resources and discharge planning needs as directed by the clinical therapist.
  • Provides discharge planning services for identified patient as directed by the physician and treatment team.
  • Contacts appropriate previous and current tertiary caregivers to obtain relevant collateral clinical data that may be needed for the patient’s coordination of care, treatment planning and discharge planning.
  • Assists with the administration of the treatment outcome package (TOPs).
  • Attends and participates in all required meetings including interdisciplinary treatment planning, administrative, in-service training, and so forth. Remains abreast of agencies, professional organizations, and other resources that will assist patients and families in providing continuity of care after discharge and directs patients and patient families to same, as appropriate.
  • Maintains knowledge of current trends and developments in the field by reading appropriate books, journals and other literature and by attending related seminars, conference and the like.

What you will need:

  • Masters Degree Social Work Required Or
  • Masters Degree Required
  • 3 Years of experience in a behavioral health care setting, with a minimum of one year being in acute care And
  • 2 Years of previous social work experience (Preferred)
  • • Ability to maintain information which is highly confidential
  • Ability to clearly summarize pertinent clinical information via written correspondence and medical records documentation
  • Ability to demonstrate self-control in volatile or hostile situations
  • Ability to set therapeutic limits and maintain therapeutic distance with clients; maintains therapeutic boundaries with patients during treatment and following discharge
  • Licensed Clinical Social Worker (LCSW) - Illinois Department of Financial and Professional Regulation (IDFPR) Required Or
