What You'll Do
- Complete comprehensive Start of Care (SOC) admissions throughout the Twin Cities Metro.
- Perform detailed nursing assessments and complete OASIS documentation.
- Develop individualized plans of care based on each patient's needs and goals.
- Educate patients and caregivers on medications, diagnoses, and home health services.
- Coordinate with physicians, therapists, case managers, and the interdisciplinary care team.
- Help ensure safe, successful transitions from hospital to home.
What We're Looking For
- Active Minnesota Registered Nurse (RN) license.
- Strong assessment, clinical reasoning, and documentation skills.
- Home health and OASIS experience preferred.
- Excellent communication and organizational abilities.
- Ability to work independently in a field-based environment.
- Reliable transportation.
