Job Summary

Job_Description_Summary
Psychiatric triage for all levels of care for McLean Hospital units on and off site, certain McLean affiliates, MGB facilities and capitated contracts. Provides teaching function for all evaluating clinicians regarding payers and precertification requirements, teaching/consulting role to junior psychiatric triage staff.

RESPONSIBILITIES AND STANDARDS

1. Facilitates inpatient admission for psychiatric and substance abuse referrals across four campuses 24/7/365.
Interviews the patient, family member or referring professional to obtain clinical and demographic data; following established admission procedure.

1.2 Gathers comprehensive and accurate information relevant to the need for inpatient admission according to established procedure, as demonstrated by minimal need of clinicians to recollect same data.

1.3 Review EPIC records for patients being referred within MGB, reviewing clinical, medical and ED timelines to review patients full clinical presentation

1.4 Triages urgent and emergent calls to appropriate level of care including referral of patients in crisis to emergency rooms when indicated and/or contacting 911 regarding patients at immediate risk.

1.5 Respond to preadmission inquiry promptly as indicated by lack of complaint.

1.6 Interacts with patients, family member or referring professional in a courteous and professional manner as indicted by lack of complaint.

1.7 With medically or psychiatrically complicated cases, reviews inquiry with Unit Nursing Supervisor or Doctor on Call or department manager in a timely and professional matter as indicated by lack of complaint.


2. Provides MGB wide bed search following established triage procedure.

Standards

2.1 Reviews clinical and financial information in EPIC according to established procedures in order to make referrals to participating MGB facilities, (when available and outside MGB when needed) based on clinical needs and managed care contracts.

2.2 Provide after hours bed finding for BWH, BWF, MGH

2.3 Act as a resource to MGB wide facilities regarding services and bed availability within MGB.

2.4 Provide psychiatric triage for Franciscan’s Children’s Hospital inpatient

2.5 Provide public relations function by handling inquiries regarding services at McLean, it’s affiliates and other MGB psychiatric facilities.


3. Continually reassess census and bed availability.

Weight: 10%

3.1 Assigns patient to appropriate unit based on census, clinical presentation at preadmission, and contract requirements across four McLean campuses

3.2 Obtains census (admissions, anticipated discharges on each inpatient unit at least three times during each twenty-four hour period and as indicated.

3.3 Maintains electronic bed board in Teams to accurately reflect real time census, expected admissions and anticipated discharges

3.4 Provides units with clinical information and expected time of arrival.

3.5 Discharges patients in timely fashion in EPIC when units leave voicemails on discharge line – accurately reflect discharges on electronic bed board in Teams and reflect census change


4. Completes financial screening according to established procedure.
