Job Summary

Status: Full-Time, Non-Exempt
Pay: $22.50-$23.00/hour
Schedule: Monday-Friday 8:30am-5pm, including some weekends, evening work, and/or availability to respond to crisis situations as needed.

Job Summary: The Housing Solutions Case Manager is responsible for helping Mercy House clients obtain and maintain permanent housing using the Housing First Model. This role provides case management services and housing advocacy for clients in our system of care. Responsibilities include assessing clients, developing and evaluating housing plans, tracking progress toward housing goals, and supporting participants in the Rapid Re-Housing and Homeless Prevention Programs.

Essential Duties and Responsibilities:
Program Oversight

  • Co-locate to assigned Regional Center office locations and provide program services in-person to HSP referrals based on need
  • Attend Family Support Team (FST) meetings as assigned
  • Provide comprehensive case management services, including intake, housing stabilization planning, goal setting, service linkage, and progress monitoring
  • Use client-centered strategies and harm reduction methods to support housing stability
  • Apply Trauma-Informed Care and Motivational Interviewing techniques
  • Maintain detailed case notes using the GIRP (Goal, Intervention, Response, Plan) format
  • Provide tenant education, advocacy, and referrals as needed
  • Develop individualized case plans, including financial planning and housing goals
  • Meet regularly with clients to track progress and ensure program compliance
  • Focus on achieving positive outcomes for clients
  • Empower clients in a supportive, objective, and client-centered manner
  • Keep case files up to date and ensure timely data entry into the HMIS database
  • Update program master logs daily with accurate client information
  • Schedule and conduct home visits as necessary
  • Complete Habitability Inspections when required
  • Assist clients with employment and housing barriers
  • Help clients identify strengths and assess needs respectfully
  • Stay informed on community resources and connect clients as needed
  • Follow all agency case management policies
  • Manage a caseload as assigned by the supervisor
  • Participate weekly in Coordinated Entry System (CES) meetings and follow all CES protocols
  • Assist clients with housing searches, provide leads, and support landlord relationships
  • Help resolve landlord-tenant issues to support housing retention
  • Manage re-certification processes and collect required documentation on time
  • Conduct follow-up appointments as directed

Team Collaboration

  • Collaborate with staff, interns, and volunteers to support the agency’s mission and core values
  • Attend staff meetings, supervision, community meetings, and trainings as needed
  • Participate in monthly Housing Solutions team meetings to review client progress and share resources
  • Coordinate with leasing agents and support clients in securing housing
  • Contribute to a positive, team-oriented environment with other case managers and leadership
