This is a full-time, 10-hour day shift role based in Torrey Pines. The position offers a hybrid schedule, requiring three in-person shifts and one remote shift per week.

We invite you to join our Hematology/Oncology team as a Specialty Nurse Navigator. As a Specialty Nurse Navigator you will be responsible for:
- Navigating patients through the continuum of care acting as a liaison and resource between the physician, other departments, and the patient.
- Establish specific, measurable, attainable, relevant and time specific nursing goals to guide the care planning process and promote self-reliance.
- Provide in person, clinic visits as needed at the initiation of care and throughout transitions of care throughout all of our hematology/oncology clinic sites, as needed.
- Formulating custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care.
- Providing disease and treatment specific education tailored to meet patient’s individual goals of care.
- Onsite rounding throughout treatment plan implementation as needed to provide needed in person patient evaluation.
- Provide direct, hands-on nursing care, as needed.
- Assist patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance.
- Assisting patients with social services needs by identifying resources in the community and referring to these programs.
- Collaboration with system wide authorizations team, scheduling in other departments, as needed to ensure patients receive timely and comprehensive care.

Qualifications
Required Education/Experience/Specialized Skills:
- BSN plus three years recent clinical nurs
