Mission: The Link works with youth and young families to overcome the impacts of poverty and social injustice. Our 23 programs provide supportive housing, intensive case management services, and skills development to youth at risk of or experiencing homelessness, sex trafficking and/or involvement in the juvenile justice system. The Link prides itself on its innovative, youth-centered, strengths-based programming for youth and families as well as programs that are co-designed, implemented and overseen by youth.
Position Specifics
- Hiring Salary Range: $23.13/$26.00/hour based on qualifications
- Job Type: Full-Time, Non-Exempt
- Location: Minneapolis
- Hours/Days: Mon-Fri - 9:00-5:00
- Note: This position requires driving. To be able to drive on behalf of The Link, candidates must have a valid driver's license, have a safe and reliable vehicle, be able to successfully clear a criminal and motor vehicle background check, and maintain personal insurance at the levels of $100,000 Bodily Injury/Per Person and $300,000 Bodily Injury/Per Accident. Job offer is contingent on having these requirements.
BASIC FUNCTION
The Link’s Rapid Re-Housing/Housing Support Program provides housing and services young people (18-24) who are exiting homelessness. The Case Managers work to support participants focusing on tenancy skills, meeting program expectations, maintaining housing, increasing academic achievement, obtaining employment, and financial literacy.
Case Managers provide a variety of assessments and evaluations and assist participants in developing Individual Service Plans and strategies for implementation, for which the case manager will monitor and provide resources, referrals, and advocacy. Case Managers also provide individualized and specialized support to participants who are pregnant and/or parenting. The Young Families Housing Support Case Manager will work with pregnant and parenting youth to support them in individual and family goals.
ESSENTIAL FUNCTIONS
- Provide case management services to clients in the Rapid Re-Housing Program (HUD) and Housing Support (formerly GRH) Program.
- Conduct assessments with clients to determine barriers, resource needs and other
- factors related to case planning, acting as a broker for services and referring them
- to community resources when necessary
- Support participants in setting and pursuing goals
- Act as liaison between client/landlord relationships relating to tenancy issues
- Assist clients in living independently
- Work with Parenting Specialist to coordinate services for clients and their children
- Assist clients in finding daycare within the county childcare system
- Support families in navigating through family courts and rights as a young parent
- Act as a liaison with other service providers
- Maintain relationship with client and provide space for client to grow and learn
- Maintain consistent home visits with clients
- Provide necessary transportation according to availability
- Assist in transporting families to groups and on field trips
- Keep updated records of all interactions with clients and any other contacts made on their behalf
- Provide on-going monitoring and reporting of participant’s progress
- Provide on-going Independent Living Skills instruction in both group and one on one settings
- Attend and contribute to appropriate weekly case review/team meetings

Requirements
KNOWLEDGE, SKILLS, AND ABILITIES
- Knowledge of child welfare systems, general and other public and federal assistance programs, and other community resources
- Experience working with Parenting youth.
- Experience working with young children ages 0-7
- Experience with parent and family county benefits such as MFIP and WIC
- Knowledge of Resources for families and pregnant youth ages 18-
