Regional MDS Coordinator

Regional MDS Coordinator (Remote Supervisory Role)
Salary: $110,000-130,000/year
SUMMARY
The MDS Coordinator is responsible for managing and coordinating the Minimum Data Set (MDS) assessment process to ensure compliance with federal, state, and company regulations. This position plays a critical role in accurately assessing resident care needs, optimizing reimbursement, and coordinating interdisciplinary care planning.

ESSENTIAL JOB FUNCTIONS

To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below:
• Coordinate and complete MDS assessments in compliance with federal and state regulations.
• Collaborate with interdisciplinary teams to develop individualized care plans based on MDS data.
• Ensure accurate documentation and timely submission of MDS assessments.
• Monitor and update assessments as needed to reflect changes in residents' health status.
• Lead and participate in care plan meetings with interdisciplinary teams.
• Train nursing and care staff on MDS processes and requirements.
• Participate in quality assurance and improvement programs related to resident care.
• Ensure compliance with Medicare and Medicaid reimbursement regulations.
• Conduct regular audits of MDS documentation to ensure accuracy and compliance.
• Performs other related duties and activities as required.

SUPERVISORY RESPONSIBILITIES
• Supervise local MDS Coordinators throughout region
• Onboard and train MDS coordinators

Minimum Knowledge and Skills required for the Job
The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job.

Education and Experience:
• Bachelor’s Degree or equivalent work experience
• LPN/LVN (Licensed Practical/Vocational Nurse) required, RN (Registered Nurse) license preferred.
• Minimum of 1-2 years of experience in long-term care or skilled nursing, with specific experience in MDS coordination preferred.
• RAC-CT Certification preferred
• PDPM experience required

Certificates, Licenses, and Registrations:
• Current RN or LPN/LVN licensure in good standing.
• RAC-CT certified preferred

Other Skills and Abilities:
• Strong organizational, leadership, time management, and communication skills.
• Familiarity with MDS 3.0 and care planning processes.
• Proficiency in electronic health records (EHR) systems and MDS software.

Other Requirements:
• Travel as needed.

Physical Requirements:
• Sedentary work. Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
