Your Specific Responsibilities:

The position is for a lead provider to manage the health care for 30 of our more complex and high-risk patients.

 

This position will act as lead medical provider for a new "Intensive Case Management Program" and will include a separate clinic, collaborating closely with the other free clinics currently at GRACE. 

We anticipate operating 4 hour in-person clinic one afternoon per week.

The lead provider will have administrative and nursing support in the clinic, and oversee continuing care provided by a nurse or paramedic working in the field to follow up, monitor and assist patients with their care plan.

 

Required Knowledge, Skills, and Abilities:

The ideal candidate should have;

- strong clinical skills

- be comfortable working in a low-resource setting, providing trauma-informed care for individuals facing significant medical and socioeconomic challenges.

 

Qualifications:

Minimum - Certification as an Advanced Registered Nurse Practitioner in accordance with Chapter 464, Florida Statutes, and in accordance with Rule 64B9, Florida Administrative Code, or certification as a Physician’s Assistant in accordance with Chapter 458, Florida Statutes.

Preferred – Experience working with high risk communities.
