What you can expect in this role:

• This is a full-time role covering an expected 40 hours per week
• The majority of this time you can expect to spend with patients
• Ascend blocks ample time on your schedule for documentation and team interactions (and of course lunch!)
• You will provide patient assessments and care via telehealth and in-office consultations
• Ability to diagnose routine conditions and consult (or refer to) physicians for more complex diagnoses.
• Treat patients by following best practice protocols and consult with physician leaders if you deem it necessary to modify the care pathways
• Communicate with your patients as necessary to ensure we offer a delightful experience.
• Manage patient treatment plan and provide support and education to patients on the way.
• Educate patients about Ascend’s broader offerings, conduct health risk assessments and guide them on their journey to longevity and happiness.

Schedule:

Monday-Friday, 8 & 10 hour shifts
