Who We Need/JOB SUMMARY (INCLUDING, BUT NOT LIMITED TO):

We are seeking a dedicated behavioral health professional to provide Supported Employment and Housing Services for members with Serious Mental Illness (SMI). Responsibilities include:
  • Supporting members in obtaining and maintaining employment following SAMHSA’s Supported Employment Core Principles.
  • Assisting in housing support and placement through Housing First/Permanent Supportive Housing models.
  • Conducting intakes, assessments, and individualized case planning.
  • Coordinating with the clinical team and housing specialists to secure community-based resources.
  • Managing financial assistance funds for housing needs.

MINIMUM QUALIFICATIONS:

  • Require HS Diploma/GED plus four (4) years of full-time direct care experience in behavioral health. 
  • Bachelor’s degree highly preferred. 
  • At least 1 year of experience working with at-risk populations and housing case management.
  • Demonstrated knowledge of Motivational Interviewing and Trauma Informed Care preferred.
  • Knowledge of community resources, workforce development, and housing resources. 
  • Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire
  • Must have reliable transportation, valid AZ Driver’s License with state minimum insurance coverage and ability to meet and maintain Copa Health’s acceptable driving requirements.
