JOB SUMMARY
Infusion clinical staff pharmacist - IV certification required. Central pharmacy operation, technician supervision, IV admixture of both hazardous and non-hazardous medication oversight. Responsible for covering the weekend and the operational needs of all infusion pharmacies. Oncology or infusion pharmacy experience preferred. PRN 0.49FTE Reviews/updates medication profiles (therapies, allergies, height, weight, etc.) and is cognizant of patient charges. Reviews medication orders for therapeutic appropriateness, duplications, interactions, overall effectiveness and safety. Documents interventions (i.e., ADR). a) Directly caring for patients; b) Directly caring for human-subjects research participants; c) maintaining, modifying, releasing patient records d) Regularly maintaining, modifying, releasing or similarly affecting human-subjects research records.

EXPERIENCE AND EDUCATION

Required

  • Education
    Bachelor's Degree - Bachelor of Science (BS) degree or 
    Doctor of Pharmacy (PharmD) degree from an American College of Pharmaceutical Education (ACPE) accredited school of pharmacy 
     
  • Experience
    1 year with experience in a hospital setting required for an inpatient position 
     
  • Licenses and Certifications
    Eligible for licensure by Texas State Board of Pharmacy (TSBP) is required and Texas license with TSBP Upon Hire and 
    Texas license with TSBP within 30 days of date of hire. within 30 Days and 
    (ACPE-IV) STERILE COMPOUNDING (IV) CERT Sterile compounding (IV) certification accredited by ACPE Upon Hire and 
    (BLS) BASIC LIFE SUPPORT certification with course accredited by the American Heart Association (AHA) or American Red Cross (ARC) Upon Hire and 
    ADV CARDIAC LIFE SUPPORT course accredited by the American Heart Association (AHA) or American Red Cross (ARC) will be required for inpatient positions within 180 Days
