Lead Case Manager

Northeast Arc is seeking a dedicated and compassionate Lead Case Manager to join our team in Danvers!

Job Summary:
The Lead Case Manager oversees and manages Adult Family Care (AFC) Case Management staff, maintains a caseload, and collaborates with the AFC Program Director to ensure high-quality services and documentation.

Major Responsibilities:

• Collaborates with the AFC Program Director to assist with hiring, training, supervision, evaluation, and professional development of AFC Case Managers and/or Community Health Workers.
• Evaluates, trains, and supports AFC Caregivers in partnership with the AFC Program RN.
• Provides comprehensive onboarding for new AFC placements.
• Maintains an AFC caseload and contributes to the development, implementation, and ongoing review of the AFC Plan of Care.
• Reviews, signs, and dates AFC caregiver logs, offering feedback and education to caregivers as needed.
• Participates in all required meetings per program standards and MassHealth regulations, as well as additional meetings, trainings, and conferences as requested by AFC management.
• Performs other duties as assigned.

Qualifications:

• Bachelor’s degree, with at least two years of recent experience working with elderly or disabled adults.
• Prior supervisory experience preferred.

Job Type:

• Office and/or remote work environment
• Home visits in the community

Schedule:
Monday–Friday, 9:00 AM–5:00 PM, Flexibility to shift schedule time.

Pay Rate:
$28.00 – $29.00 per hour
