Description

Position Summary -

The Performance Improvement and Patient Safety Coordinator plays a vital role in advancing the hospital’s mission by leading, supporting, and coordinating initiatives that improve patient safety, healthcare outcomes, and system efficiency. This individual serves on the hospital’s Performance Monitoring and Improvement Committee and acts as a resource and advisor to departments on continuous quality improvement (CQI) efforts.

 

Key Responsibilities:

  • Performance Improvement & Safety Oversight:
    • Conduct root cause analyses (RCA) under the supervision of the Director of Quality.
    • Conduct process confirmation rounds to ensure sustained improvements and adherence to protocols.
    • Perform real-time and as-needed patient safety evaluations, escalating and addressing issues proactively.
    • Provide evidence-based recommendations aligned with hospital policies when process, structure, or outcome concerns are identified.
    • Review and support departmental and hospital-wide PI projects, ensuring alignment with regulatory standards and strategic goals.
  • Committee Leadership:
    • Serves on various quality management committees, ensuring effective agenda-setting, documentation, follow-up, and multidisciplinary engagement.
    • Facilitate data-driven discussions and promote accountability for improvement initiatives.
  • Patient Safety & Risk Reduction:
    • Collaborate with clinical and non-clinical teams to identify, investigate, and address safety risks.
    • Analyze trends and incidents to drive proactive interventions that reduce harm and improve outcomes.
    • Promote a culture of safety through visibility, coaching, and team engagement.
  • Education & Capacity Building:
    • Provide training and support on performance improvement and patient safety methodologies, tools, and best practices.
    • Educate staff and leaders on regulatory requirements and support the development of quality-related competencies across departments.
  • Data Analysis & Reporting:
    • Assist with reviewing clinical, operational, and patient experience data to identify opportunities for improvement.
    • Support the development and presentation of dashboards, scorecards, and summary reports for stakeholders and committees.
    • Partner with informatics and analytics teams to ensure meaningful, actionable data.

 

Qualifications:

  • Bachelor’s degree in Nursing, Healthcare Administration, Public Health, or related field required; Master’s preferred.
  • 2-3 years of experience in healthcare quality, performance improvement, or patient safety; 3-5 years preferred. 
  • Working knowledge of regulatory standards (e.g., CMS, DNV and quality improvement tools (e.g., PDSA, RCA, FMEA).
  • Proficiency with Microsoft Office Suite (Word, Excel, Outlook, Teams) and comfort with electronic recordkeeping.
  • Experience facilitating interdisciplinary teams and conducting real-time safety reviews.
  • Certification in CPHQ, CPPS, Lean Six Sigma, or related field preferred or willingness to obtain within one year.
  • Strong interpersonal, analytical, and organizational skills.
