Purpose

The Director of Referral Coordination and Navigation, Nursing leads the strategy and daily operations behind seamless patient transitions, efficient referrals, and integrated navigation services across a complex health system. Partnering with leaders in Patient Access, Health Information Management, and Social Work, this role ensures patients move smoothly through every step of their care journey. Reporting to the Executive Director of Ambulatory Clinics, Patient Access, and the ASC, the Director works closely with clinical and administrative leaders to align care pathways, support front-line staff, and uphold high standards of compliance and performance. This role is part of a dynamic leadership team alongside peers in Patient Access, Nursing, and Revenue Cycle.

Responsibilities

Manage Referral Processes

  • Coordinates the development and implementation of referral policies and procedures in partnership with the Director of Patient Access and Health Information Management.

  • Oversees the nursing referral coordination team to ensure timely and accurate patient referrals.

  • Monitors referral metrics and reports to senior leadership.

Coordinate Patient Care

  • Collaborates with clinical staff to ensure coordinated patient care across departments.

  • Develops care plans and pathways for complex cases and case management services in collaboration with the Health Social Work leadership team.

  • Ensures compliance with care coordination standards and regulations.

Navigation Services

  • Leads the nursing patient navigation team to assist patients in understanding their care plans.

  • Provides training and resources for navigators to support patient needs.

  • Evaluates navigation program effectiveness and implements improvements.

Staff Management

  • Hires, trains, and supervises referral coordinators, case managers and navigators.

  • Conducts performance evaluations and provides professional development opportunities.

  • Manages team schedules and workload distribution.

Policy Development and Compliance

  • Develops and updates policies related to referrals, coordination, and navigation.

  • Ensures compliance with healthcare regulations and standards.

  • Prepares reports and documentation for regulatory bodies.

Required Qualifications

  • Bachelor of Science in Nursing (BSN)

  • 5 years of relevant nursing experience, including 3 years in a leadership role

  • Texas Registered Nurse (RN) or compact license.

Preferred Qualifications

  • Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)

  • 7 years of relevant nursing experience, including 5 years in a leadership role

  • Certified Case Manager (CCM)

  • Certified Professional in Healthcare Quality (CPHQ)

  • Bilingual English/Spanish
