20-hour 100% onsite social work position to provide a range of clinical social work services and case management to patients in the lung transplant program. This social worker will care for the organ group population in all phases of transplant (evaluation, transplant admission and follow up).

This Transplant Clinical Social Worker will be responsible for:

• Psychosocial assessment as part of transplant candidacy
• Counseling and crisis intervention
• Facilitation of use of hospital and community services
• Resource information and referral, and program development
• Psychosocial consultation to multidisciplinary team

To qualify, you must have:

• A Master’s degree in social work
• LICSW Licensure in state of MA
• Flexibility and ability to work in fast-paced, dynamic environment essential.
• Excellent communication skills necessary.
• Experience in medical environment, bereavement, child protection issues essential

This position is 100% onsite, and eligible for part time benefits
