Tri-Cities Community Health (TCCH) is looking for a Community Outreach Health Worker who has a passion for helping others. As a community health center, located in southeastern Washington, serving Pasco, Kennewick, Richland and the surrounding communities.

Who are we?
Our community health center began as a modest sized medical clinic in Pasco, Washington in 1981. The clinic was founded by a small group of women in the Pasco community who saw the need to serve low-income community members who could not afford to pay for healthcare.
Tri-Cities Community Health has grown into a comprehensive non-profit Federally Qualified Health Center (FQHC) where we have evolved into a network of community health centers that continue with the foundation TCCH was built on and serve an even more diverse population in the Tri-Cities and surrounding areas.
Our Opportunity:
As a Community Health Worker (CHW) at Tri-Cities Community Health, a Federally Qualified Health Center (FQHC), you will play a vital role in improving the health and well-being of the individuals and communities we serve. Working closely with patients, families, and community organizations, you will serve as a bridge between healthcare providers and the community, promoting access to quality healthcare services and addressing social determinants of health. As a member of the Tri-Cities Community Health team, you will have an opportunity for a rewarding position that can positively impact the health and well-being of the underserved members of our communities and contribute to reducing health disparities and promoting health equity.
Essential Functions:
- Conduct Outreach activities such as community presentations, health fairs, and workshops to raise awareness about our services and health promotion initiatives
- Utilize managed care rosters to identify and engage individuals and families in the community who may benefit from our healthcare services.

- Collaborate with community organizations, schools, and other stakeholders to establish partnerships and build strong referral networks.
- Assist patients in navigating the healthcare system by providing guidance on insurance enrollment, appointment scheduling, and accessing appropriate resources
- Conduct one on one or group sessions t teach individuals about healthy lifestyle choices, disease prevention, and self management techniques
- Assist individuals in setting health goals, developing action plans, and monitoring their progress toward achieving optimal health outcomes
- Collaborate with healthcare providers, case managers, and social workers to ensure seamless coordination of care, improve patient engagement, and reduce hospital readmissions.
- Facilitate referrals to community resources such as housing assistance, transportation services, food banks, and mental health programs.
- Maintain accurate and up-to-date records of patient interactions, interventions, and outcomes using the electronic health record system.
- Collect and document relevant data related to program activities, community needs, and patient outcomes.
- Compile reports and summaries to evaluate the impact of community health programs and identify areas for improvement.
- Participate in quality improvement initiatives by providing feedback and suggestions for enhancing service delivery.
- Ability to carry out other duties as assigned
Schedule: Mon- Fri 8:00am - 5:00pm

Wage: $20.54 - $26.57 per hour

Requirements
Minimum Education:
- HS Diploma
- Associate or Bachelor in the related fiel
