The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical, emotional, and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities.

 

• The social worker in this position is based in the integrated cardiology practice and provides psychosocial assessments and a full range of psychosocial interventions, both individually and in groups to patients with cardiac diseases. 

• Responsibilities include conducting clinical assessments and coordination of psychosocial components of patient care. 
• In addition to the counseling component, interventions include assistance with concrete services such as entitlements, home care, pharmacy and transportation needs.  
• The social worker will also provide daily coverage to the Cardiac Catheterization Lab at Mount Sinai Heart, providing discharge services including transportation, home care and information on available resources within the community or hospital. 
• The social worker is also responsible for social work program development within the Cardiovascular Institute.  
• In addition, the social worker will take a leadership role in working with a multidisciplinary team overseeing the practice and act as a link between medical staff and patients. 
• The social worker will have responsibilities that include committee meetings, quality improvement initiatives and may also include early morning or early evening meetings. 
• There is a strong focus on program development and patient satisfaction.

 

• NYS LMSW or LCSW. 
• Experience in providing rapid psychosocial assessment and focused, short-term treatment.
• Knowledge of a range of entitlements, resources and the ability to assist patients with concrete needs and provide linkages, referrals, and enhanced patient follow-through by reducing barriers to care.
• Ability to assess needs and coordinate resources with hospital and community networks for effective management of patient care.
• Excellent clinical, collaborative, organizational, and leadership skills.
• Bilingual (English/Spanish) preferred.
• Minimum one year in current position unless in Cardiac.

 

Compensation Statement

 

The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $85,280.00 - $97,760.36 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
