Department Overview

The Health Equity department is comprised of functional leaders and support staff who work together to advance health equity for OHSU Health. Centering the highest quality of health for all, the Health Equity org. partners across all communities, sectors, and regions of the state to recognize, reconcile, and rectify historical and ongoing health injustices stemming from bias, racism and other social and structural factors that influence access to healthcare and contribute to poor health outcomes. The Health Equity department does this through data evaluation, integration of health equity efforts across OHSU Health, provision of health services through Place-Based health model, and the development of transformational partnerships within the community.

The community mobile health program manager is an operationally-focused program management position responsible for executing clinical objectives for the Health Equity department’s “Place-Based Health” program. Place-Based Health is a community-centered clinical operation that provides primary care health services to community members in spaces where they feel a stronger sense of belonging (e.g., community-based organizations) throughout the state. In doing so, Place-Based Health aims to advance health equity by improving quality outcomes and reducing barriers to access for marginalized and underserved communities.

The program manager will directly supervise staffed and volunteer clinical support teams including (but not limited to): medical assistants, PAS staff, community health workers, RNs, LPNs, etc. The program manager will work closely with the Health Equity team members and leadership to ensure operational readiness for all off-site community-based clinics and events which includes supply ordering and procurement, staff training, scheduling, EMR stewardship, and workforce communication. The program manager ensures that all state and internal regulatory and compliance guidelines are adhered to and takes necessary corrective action when gaps are identified. The individual will execute clinical operations in various off-campus locations in partnership with community-based organizations. These sites will either be recurring or non-recurring, and will require a strong application of project management tools and the ability to implement various clinical models to best serve the needs of each population.

The program manager will be responsible for hiring, coaching, and directly managing represented clinical support team members in accordance with contractual guidelines. The individual will prioritize community needs, while also working with competing priorities, limited resources, and sometimes limited timelines. Most importantly, the person in this role will apply a social justice lens to establish and maintain relationships with a diverse group of community members, staff, and volunteers that acknowledges power dynamics and flattens hierarchies, in order to achieve optimal results.

Function/Duties of Position

Clinical Operations Management

  • Provides on-site management of operational activities and supervision of the clinical team for all community-based clinical activities in the Place-Based Health program and within the Health Equity portfolio.
  • Manages master Epic templates for Place Based-Health and mobile health coalition. Assigns provider/department templates per department guidelines.
  • Procures clinical supplies and ensures operational readiness of mobile van and all associated equipment needs prior to each clinic.
  • Demonstrates knowledge of standards developed by organizations monitoring patient care including, but not limited to, DNV and CLIA, in conjunction with operations director, coordinates patient flow.
  • Identifies potential problem areas, develops a system for objectively monitoring performance, and creatively seeks solutions to foster quality improvement.
  • Provides timely and appropriate resolution to patient, customer, physician, personnel, vendor, and organizational concerns.
  • Demonstrates flexibility and continuously re-evaluates the need to adapt operational models and initiatives based on needs of the community. Ensures that these adaptations are in line with OHSU, and within guidance of OSHA, OHA, CDC, UTT and all government regulations.

Project/Program Management

  • Initiates, plans, executes, and closes discrete operational projects successfully within the Health Equity Place-Based Health program.
