The Community Health Program Manager develops and implements both systemwide and region-specific strategies that promote community education, outreach, and population health.
Serving as a community ambassador for their assigned medical center, the Program Manager represents Atlantic Health System (AHS) in key relationships with local organizations and health coalitions, fostering collaborative partnerships.
This role provides oversight of the triannual Community Health Needs Assessment (CHNA), the annual Community Health Improvement Plan (CHIP), and supervises health education staff on specific projects and initiatives.
- Lead the implementation of systemwide Community Health initiatives and oversee deployment of programs and community engagement activities within the assigned medical center region.
- Support the department’s health education strategy by coordinating culturally sensitive, evidence-based programs and health promotion materials on priority health issues across the lifespan, in alignment with the Community Health Improvement Plan.
- Manage the outreach strategy for the assigned service area through health and wellness events, promotional tools, collaborations with community partners, and targeted outreach to populations experiencing health disparities.
- Oversee involvement in the triannual Community Health Needs Assessment and annual Community Health Improvement Plan and actively participate in Community Health Advisory Committees and Community Advisory Board meetings.
- Develop and maintain collaborative relationships with internal departments and community partners by participating in hospital committees and community-based coalitions focused on population health priorities. Represent AHS as the “face” of Community Health for the assigned region.
- Provide supervision and support to the health education team on assigned projects and initiatives, including directing workflow, monitoring progress, and ensuring timely achievement of goals.
- Assist in managing community health budgets, expenses, and revenue tracking, and support grant oversight and reporting for funded initiatives in the service area.
- Lead tracking and reporting efforts related to community health programs, including outreach and event data, evaluation outcomes, health improvement impact, grant deliverables, and community benefit reporting.
- Collaborate with AHS Care Coordination to address barriers to care and improve navigation across the care continuum by connecting patients and communities to appropriate health and social resources.
- Perform other duties as assigned by the Community Health Director or Manager to support departmental goals.
Education:
- Bachelor’s degree in Public Health, Health Education, or a related health field required.
- Master's degree with a concentration in Public Health, Health Education, or a related health field preferred.
Experience:
- 3-5 years of experience in public health, health education, or a related health field preferred.
- Certified Health Education Specialist preferred.
- Certified Tobacco Treatment Specialist (CTTS) a plus.
- Association of Diabetes Care & Education Specialists (ACDES) training a plus.
- Excellent oral and written communication skills.
- Strong organizational skills and ability to multitask effectively.
- Strong understanding of health and social issues affecting diverse communities, with a high level of cultural competence.
- Proficient in Microsoft Office applications.
- Bilingual in Spanish and Creole a plus.
- Valid NJ Driver's License.
- Evenings and weekend availability is required.
