Serves as the charge pharmacist for a specific pharmacy location. If applicable, this individual will hold any site-specific licensing (DEA, BOP, etc.). This individual possesses a deeper knowledge of processes and operations related to their site and often serves as the primary contact for site-specific staff and pharmacy/nursing leadership. This includes ownership of the pharmacy space to make sure it meets all regulatory requirements. Actively engages in and leads appropriate hospital and/or departmental committees relevant to site and assists with creation and implementation of workflows related to their site. Responsible for all aspects of the Staff Pharmacist 2 role in addition to further involvement in teaching, educational, supervisory, and quality improvement activities within operational practice area. Responsible for verifying new medication orders, checking prepared medications, overseeing dispensing from the pharmacy, and completing clinical patient review for safety and accuracy. Provides safe, efficient, high quality, patient-centered, and evidence-based medication services to all patients. In cooperation with other pharmacists and pharmacy technicians, acts as a resource and provides answers to drug information questions for patients and family members, pharmacists, nurses, physicians, advanced practice providers, medical students, undergraduate and graduate pharmacy students, and other associated hospital personnel. Screens for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food, drug-disease state, and drug-laboratory interactions. Engages in appropriate hospital and/or departmental committees relevant to practice area.
Position Summary
This position functions to coordinate all aspects of patients' pharmaceutical needs at The Ohio State University Wexner Medical Center with additional responsibility to act as representative of their site through involvement in teaching, educational, supervisory, and quality improvement activities Duties include, but are not limited to, medication order approval and distribution; assurance of appropriate dose, route, frequency, and duration of therapy; clinical profile review to assess safety and accuracy of medication services; patient medication profile review for drug-drug interactions, duplication of therapy, drug allergies and appropriate regimens; coordination of patients' pharmaceutical needs with physici
