SUMMARY:

Hours:

Monday- Friday: 8:00am-4:00pm

SPECIFIC DUTIES AND RESPONSIBILITIES: All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA).

  • Provide and document individual, family and group activities directed at solving daily problems related to community living and interpersonal relationships.
  • Facilitate or co-facilitate psycho-educational skill building groups pertaining to the alleviation and management of psychiatric and/or substance abuse disorders and document progress.  Teach ADL, Illness management skills, and community re-integration skills.
  • Assess and document significant medical, psychological, and social factors contributing to person served and inform other service providers involved on a timely manner to coordinate the appropriate services.
  • Record adjustments and progress regarding finances, employment, food, clothing, housing, physical and mental health well-being.
  • Acknowledge and document all behavior(s) on the part of the persons served that is considered inappropriate or a cause for concern.
  • Attempt to redirect any person in a respectful and appropriate manner, offer supportive counseling and utilize crisis intervention techniques & contingency plans whenever necessary based on the need and staff assessment.
  • Record and monitor individual recovery plans and perform follow-up to determine quantity and quality of service provided to the persons served.
  • Formulate SMART recovery plans on individuals, complete monthly note, weekly progress notes as well as 40 hours a month of MRO skill building for all participants.
  • Complete monthly group schedule based on client needs – 40 hours a month
  • Oversee recreational activities
  • Assist with screenings and referrals
  • Develop and maintain positive community relationships
  • Advocate on behalf of persons served in receiving the required services needed to become successful and satisfied in the community.
  • Establish and maintain cooperative relationships with community resources, service providers and other organizations.
  • Assist with staff scheduling
  • Assist with department staff orientation and training
  • Assist with providing support to department staff
  • Assist the program director with development of systems and implementation.
  • Assists with on-call duties.
  • Conduct chart audits and assure program adherence.
  • Oversight of inventory and ordering of supplies
  • Assume all other appropriate duties as deemed necessary by the supervisor.
  • Assist with program Development and new initiatives to improve services.

 

QUALIFICATIONS:

  • Knowledge of principles and techniques of basic counseling, crisis intervention, and trauma.
  • Understanding of physical, psychological, social and economic effects of substance abuse, homelessness, trauma, medical and mental health disorders.
  • Strong knowledge of mental health disorders, and interventions.
  • Strong knowledge of rehabilitative skill building
  • Ability to facilitate groups and provide rehabilitative services
  • Knowledge of principles of trauma informed care. 
  • Ability to establish and maintain cooperative professional relationships with clients, colleagues and supervisors.
