Ā Position Hours: Sunday- 7am-3pm, Monday-Thursday- 3pm-11pmĀ 


Qualifications: Must be at least 21 years of age. Must possess a high school diploma or equivalent. College background with related courses and related work experience preferred. Must have valid driver's license with good driving record. Upon employment must pass all mandatory Division of Developmental Disabilities trainings and maintain current certification in First Aid and CPR, Handle with Care, and other trainings as required by Our House, Inc., the Department of Developmental Disabilities, and Medicaid.


Job Purpose: Trains the residents, who are individuals with developmental disabilities, in independent living skills in group homes/apartments, which are located in single family and multi-family homes. Assists the Site Manager in managing the residence and supervising staff. Assumes the Site Manager's responsibilities during the Site Manager's absence.


Requirements

  • Trains and supervises residents in the development/enhancement of independent living skills in such major areas as nutrition, meal planning and preparation, household skills, clothing maintenance, personal grooming and hygiene, money skills, recreational and social skills, safety, travel training, sexuality training, self- advocacy and other areas.
  • Responsible for the safety and well-being of the residents.
  • Supervises the administration of resident medication in accordance with Our House, Inc. and Division of Developmental Disabilities medication procedures.
  • Transports residents in agency vehicle to appointments, shopping, recreational activities, etc.
  • Assists with the supervision of residential staff. trains, delegates assignments, monitors job performance, provides input on staff evaluations, develops staff potential and communicates changes/needs to the Site Manager and staff.
  • Assists with residence management: Assists the Site Manager with home/apartment and resident logs, social and recreational planning, menu, budgeting and spending, home and vehicle maintenance, ensures that weekly house/apartment meetings are held.
  • Assists Site Manager in residential development by helping to set and achieve residential goals (Annex A).
  • Completes assignments from the Site Manager.
  • Complies with Our House, Inc. (OHI) and Division of Developmental Disabilities (DDD) standards in all aspects of job performance according to OHI Standard Operating Procedures.

  • Incorporates OHI philosophy in all aspects of job performance.
  • Completes all home/apartment and resident logs in an accurate, thorough and timely manner. Completes and distributes incident/accident reports.
  • Assists residents in developing and following menu, chore/household responsibilities and weekly activity profile.
  • Accounts for residential funds and spending. Obtains receipts, keeps records and follows-up on reimbursements. Follows budgetary guidelines.
  • Attends and participates at monthly staff meetings. Conducts the meetings in the absence of the Site Manager or as assigned.
  • Attends and participates in all agency staff meetings.
  • Maintains the safety of the home, property and vehicle by following general safety precautions, maintaining security at all times, immediately correcting unsafe conditions, properly maintaining fire alarm system, conducting emergency evacuation drills as assigned and promptly removing snow/ice.
  • Requirement to wear Personal Protective Equipment (PPE), including a mask, as directed by CDC, OSHA, state and local authorities. All employees must wear masks, as directed, while working their shifts. PPE requirements are subject to change at any time.
  • Addresses regular home maintenance needs as they occur or reports major needs to the Site Manager. Reports emergency maintenance needs to the Director of Residential Operations and Maintenance. If after regular business hours, reports emergency maintenance needs to the On-Call person.
  • Assists Site Manager in developing and establishing resident goals in conjunction with Interdisciplinary Team through the the IHP/ISP.
  • Monitors residents' progress toward IHP/ISP goals and documents progress on daily progress notes.
  • Advocates for the rights of the residents.
  • Identifies and meets special/individual needs of the residents, such as, physical, psychological, social, religious, civil, legal and financial needs.
  • Observes residents for health problems and responds to medical needs. Sets or assists residents in setting regular medical/dental appointments.
  • Monitors and accounts for resident funds and clothing allowance.
  • Develops community resources and assists residents in developing community resources.
