Description

 

The Department of Mental Health, Western Mass Area Children’s Division, is seeking a qualified professional for the position of Human Services Coordinator II to promote services that are clinically appropriate, least restrictive and delivered close to home.  The services are designed to be strength-based, recovery-oriented and culturally/linguistically competent. The Human Services Coordinator II will provide direct supervision to Human Services Coordinator I staff who work with children and young adults in the community or in a hospital setting.


The selected candidate will participate in DMH eligibility determination for continuing care process, assign and review cases referred to case management, be responsible for client assessment and development of Individual Service Plans, provide consultation to staff and service providers and seek consultation when necessary.  The selected candidate will have personnel functions such as interviewing and recommending job applicants, completing performance evaluations and be responsible for the efficient and effective utilization of MHIS database.


Please note:  This is a full-time, 40 hours/week position in the Children's Division (Franklin County).  The schedule for this position is 8:30 a.m. to 5:00 p.m. Monday through Friday with Saturday/Sunday off.


Duties and Responsibilities (These duties and responsibilities are not all inclusive):

  • Supervises Human Service Coordinator I staff.  Provides regular case supervision and case consultation to each case manager under his/her direct supervision.  Assists directly with problematic inter-intra agency coordination and referral.  Aids case management staff in assessment, referral, and advocacy of clients to ensure regular and consistent support to supervised staff, makes clinical recommendations, oversees development of Individual Risk Management plans for individuals in need on supervisees caseload, reviews and signs off on requirement documentation ensuring they meet required standards and timeframes. 
  • Assesses work performance of case managers directly supervised by incumbent in EPRS reviews, semi-annual reviews, and annual reviews to ensure quality of work which meets job expectations.
  • Performs computerized documentation as required in MHIS; monitors MHIS data entry for assigned staff and oversees corrections, arranges for program billing numbers as required, authorizes service provision and completes service authorization for DMH continuing care purchased services, and approves case management billing.
  • Assists the CYF Director or designee in screening and hiring new staff: Human Services Coordinator I & II and support staff to ensure recruitment of qualified candidates.
  • Assigns new cases to Human Services Coordinator I staff who are directly supervised by incumbent, prioritizing client's needs to ensure adequate coverage of all cases assigned to supervised staff.
  • Provides on-going and regular supervision meetings, e.g., clinical reviews, staff meetings, in order to ensure a high quality of service delivery to clients.
  • Provides on-the-job training and orientation for employees to ensure that new employees are adequately exposed to procedure, policy, and protocol in case management service care across all area services.
  • Participates with the CYF Director as requested in the planning, development, and implementation of case management processes, practices and procedures and eligibility determination process, practices, and procedures.
  • Responds to service providers, in-patient units, crisis, respite programs as identified contact person in supervisory role.
  • Attends and participates in hospital/provider meetings to ensure communication of clinical, service planning data and engages in problem solving activities. (e.g., residential utilization review meetings, crisis manager’s meeting, respite meetings, and program collaborative meetings).
