Position Description:
 

The Wellness for Life Nurse Practitioner will assist with wellness visits and create or update personalized care plan to prevent or manage illness based on current health and risk factors. This will include performing detailed assessment, reviewing family and past medical history to assist in developing personal health and wellness goals. The Wellness Nurse Practitioner will work closely with the Director of Employer and Consumer health as well as the Wellness for Life team amongst all wellness programs. This role can include but not limited to clinic, telehealth, and community based initiatives.

Qualifications:
1. Current Advanced Practice Nursing licensure by the Louisiana Board of Nursing required.
2. Experience working in a community health care setting preferred.
3. Knowledge of community resources preferred.
