About the role
- Respond to incoming calls from patients, families, and caregivers; assess needs based on symptom presentation.
- Offer clinical guidance for symptom management using active listening and clinical judgment.
- Triage and escalate cases requiring in-person visits, urgent interventions, or emergency services.
- Communicate pertinent updates to the RN case manager, physician, and interdisciplinary team.
- Coordinate urgent delivery of medications, supplies, or equipment when needed.
- Notify leadership of high-risk situations or adverse events promptly.
- Document appropriately in EMR all triage calls accurately and in a timely manner, including assessments, interventions, and follow-up needs.
- Provide emotional support and education to patients and families navigating end-of-life.
- Other duties as assigned.
