JOB SUMMARY:. The Team Supervisor is responsible for ensuring that care and services are delivered appropriately as well as for the administrative supervision of assigned personnel.
Qualifications
REQUIRED EDUCATION & EXPERIENCE:
Must be a graduate of an accredited practical nurse or vocational nursing program
Must be currently licensed as an LPN/LVN or higher in the State
Must have previous experience in a hospice/home care setting
Must have two (2) years of management, coordination or supervisory experience
REQUIRED KNOWLEDGE, SKILLS & ABILITIES:
Must have a proven ability to work within an interdisciplinary setting
Must comply with accepted professional standards and practices
Must have excellent observation and communication skills, and good clinical judgment
Must understand hospice philosophy and issues of death/dying
Must understand principles of pain symptom management
Must demonstrate excellent observation, verbal and written communication skills
OTHER REQUIREMENTS:
Must be flexible in work hours
Must possess a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
ESSENTIAL JOB FUNCTIONS:
Assign appropriate hospice personnel to referral cases as needed
Review and coordinate with the RNCM on cases through a variety of means such as home visits, conferences, records review and the services provided by clinicians
Discuss and verify impressions and guide assigned personnel to promote more effective performance and delivery of quality services in coordination with Director of Patient Care Services
Screen and interview applicants and make recommendations for hire to the Director of Patient Care Services and Executive Director
Complete orientation of new employees per the orientation process while ensuring that competencies are met in collaboration with the Director of Patient Care Services.
Collaborate with the Director of Patient Care Services and other supervisory hospice personnel in the planning, implementation and evaluation of in-service and continuing education programs
Implement organization policies, procedures, goals and objectives both short- and long-term
Comply with accepted professional standards, accreditation, regulations and practices
Participate in public relation and community activities that promote the organizations role as an effective member of the hospice and health care delivery system
Promote customer service orientation to all hospice personnel
Participate in quality assessment performance improvement teams and activities as directed by the Director of Patient Care Services in accordance with the
QAPI Program
Provide general oversight of the CNA Programs day-to-day operations with staff scheduling, onboarding, orientation, ongoing education & skills, community concerns, staff morale and weekly meetings in collaboration with the DPCS
Provide input and coordinate up-to-date evaluations for 90-day reviews and annual reviews. This may include ongoing annual competencies required for assigned staff members, assuring completion by either the RN Case Manager, CNA or the appropriate peer staff (MSW, SC)
Attend and ensure that appropriate education for all CNAs occurs in which a Registered Nurse is required to be present, most specifically the monthly CNA meetings
Perform other duties and activities as delegated by the Director of Patient Care Services
WORK ENVIRONMENT:
Must comply with company policies, procedures and processes in addition to Hospice and Homecare rules and regulations
Must be dependable and flexible in work schedule
Must comply with harassment and discrimination policies
Must foster good working relationships
Must possess and maintain current CPR certification
Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements and is in good working order
Must be willing to submit to a criminal background check as well as a drug/alcohol screening upon hire and periodically thereafter
PHYSICAL ENVIRONMENT:
Must be able to see color changes on a patient's skin; can listen to heart and lung sounds through a stethoscope; and feel skin temperature, as well as palpating pulses
