Santa Fe Recovery Center/Four Corners Detox Recovery Center is a leading provider of substance use disorder and mental health treatment and education services. We are looking for a full-time Street Outreach Coordinator to join our team at our Gallup offices. We offer a competitive salary range from $65,000 - $60,000 (BOE) and the Benefits shown below. Summary:
The Street Outreach Coordinator, under the supervision of the Director of Prevention & Recovery, is responsible for leading outreach initiatives, managing diverse projects, and working with at-risk populations. A key aspect of this role is coordinating outreach efforts while ensuring comprehensive data collection to measure performance and inform decision-making. The Coordinator will also facilitate consortium collaboration to expand access to services and enhance delivery. This position is critical for partnering with the community consortium to deliver healthcare services, demonstrate improved health outcomes, and ensure sustainability through data-driven strategies.
Duties and Responsibilities of the Street Outreach Coordinator: (Included, but not limited to)
You’ll report to the Director of Prevention and Recovery and:
- Coordinating the accurate collection and maintenance of service data, including Electronic Health Records (EHR), intake documents, and demographic records.
- Works closely with the evaluator to gather, analyze, and report data as required and by federal deadlines.
- Ensuring the achievement of program goals and objectives by overseeing project activities, monitoring progress, and implementing necessary adjustments.
- Attends and co-facilitates HRSA Grant Technical Support meetings and other meetings and trainings relevant to street outreach and consortium initiatives.
- Helps to promote and maintain a culturally responsive environment by effectively working with individuals or groups representing diverse backgrounds, interests, and points of view.
- Partners with MAI/MAT Coordinators on outreach initiatives like rapid testing or mobile MAT through the utilization of community engagement and evidence-based or innovative, evidence-informed models.
- Coordinates and co-facilitates meetings with consortium partners including increasing formal partnerships and collaboration.
- Support colleagues and partners by providing strong leadership, expectations, and leading by example.
- Provide guidance, direction, and oversight to ensure team members meet performance expectations, maintain productivity, and adhere to organizational policies. Conduct regular check-ins, performance evaluations, and address any staff concerns or issues that may arise.
- Attend events where booth set up is required and heavy lifting, up to 25lbs, is needed.
- May drive a van/bus; complete daily vehicle inspection report, keep van/bus clean, and maintain all safety conditions in the van/bus.
- Maintains ethical and confidentiality rules and regulations in accordance with SFRC policies and procedures and HIPAA and 42CFR Part 2.

Requirements
Skills/Knowledge/Abilities:
- Excellent customer service skills.
- Detail-oriented and organized.
- Thorough knowledge of reference and research methods and techniques used in collecting, compiling, and organizing data.
- Computer literate, including advanced knowledge of Excel and experience with EHR & CRM systems.
- Works well with diverse populations.
- Ability to problem solve and consistently meet reporting deadlines.
- Familiarity with other community-based organizations and continuum of care.
