Remuneration: $64.79 - $70.22 p/h + 11.5% Super + Not for Profit Salary Packaging + Employee Benefits

An integral part of our team at The Wesley Hospital as a Social Worker - Team Leader - Acute Setting , you’ll be encouraged to grow, develop and feel empowered to make the everyday easier for the people we care so passionately about.

Your part in The Wesley Hospital team

As the Team Lead your role will consist of both clinical and non- clinical responsibilities including clinical governance, Service delivery, quality initiatives and education through an acute hospital setting.

You will supervise a team of six Social Workers and partner with our discharge planners. You will manage a diverse case load ranging from future planning, life transition for chronic disease, grief and loss adjustment to illness and disability, crisis and conflict management as well as interventions for a range of individual and social challenges. 

With a commitment to patient advocacy, you will apply holistic and integrated care management plans and complete comprehensive psychosocial assessments, focused on early intervention and prevention. You will work towards patient goals for reintegration into the community by utilising your partnerships and providing a valuable link between support services and patients.

Your interpersonal skills allow you to build strong relationships and credibility with patients, enabling greater effectiveness in your case management and a history of successful patient outcomes. You have a proven ability to deliver a high level of clinical reasoning and decision making that supports your commitment to patient advocacy.

What makes a difference for us?

You will have a well-rounded knowledge of best practice approaches to patient care, including up to date knowledge of the government’s My Aged Care and NDIS initiatives as well as:

  • Bachelor of Social Work or equivalent
  • Eligiable for membership with AASW
  • Minimum of 4 years previous experience in acute Social Work within a hospital environment
  • Supervisory experience desired
  • Experience with DV caseloads desired
  • Ability to manage complex and variable caseload
  • Commitment to ongoing professional development
  • Involvement in quality and service improvement initiatives 
  • Must hold or be eligible to hold a current Blue Card
