Compensation Range:

$63,003.00 - $80,600.00

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

GENERAL PURPOSE
The Social Worker is responsible for providing clinical services to patients who are admitted to GCH 7N. Clinical services are also provided to the families of these children.

ESSENTIAL FUNCTIONS

• Provides risk evaluations for patients identified as being at psychosocial risk, followed by assessments utilizing the SWK pediatric psychosocial template of patients requiring SWK interventions.
• Provides SWK interventions and supportive counseling to patients and families that address risk factors influencing health outcomes. This includes support to families receiving new chronic illness diagnosis, adjusting to a life-threatening illness or disability, and support around end of life decision making and bereavement.
• Serves as a resource consultant to patients and families and offers linkage to other health and community services as needed.   
• Provides counseling for patient and families around special Medicaid programs and supplemental insurance.
• Documents SWK activity in the patient’s medical record including completion of the psychosocial assessment, palliative care and substance abuse assessment as appropriate.
• Provides discharge planning for the patients, assumes primary role when there is CPS involvement, alternate D/C of a minor or transfer to another facility. Provides handoffs to ambulatory, other pediatric social works to ensure continuity of SWK care.
• Attends interdisciplinary rounds and collaborates with the team, serves as liaison to health and social service agencies involved in the care of the patient.
• Identifies and assists in managing risk situations including disruptive visitors, in close consultation with Pediatric clinical manager SWK Division.
• Identifies and manages all cases of suspected child abuse and neglect not covered by other pediatric specialty social workers
• Provides concrete services to families needing assistance with housing, transportation, prescriptions and meals during hospitalization
• Carries out other clinical/program tasks associated with the position of social worker for GCH 7N
• Other duties as assigned
• Participates in activities of the SWK Division and the GCH 7N including; attend staff meetings, time reporting, professional development and mandatory in-services.
• May serve as a member of Division or Unit Committees
• May serve as a SWK field instruction supervisor
• May provide consultation and teaching to Hospital staff on psychosocial issues in the health care of children.

QUALIFICATIONS

• Must hold a Master’s degree from an accredited institution.
• Experience in a pediatric medical environment and work with an interdisciplinary team preferred.
• or equivalent combination of education and experience preferred.
• Licensure required within 18 months of hire required.
