Description
CPC is seeking candidates for our Self-Sufficiency Coach positions in York, PA.SUMMARY: Serves as the liaison between the agency, the community, and clients. Provides intensive coaching to clients in York County to assist them in achieving self-sufficiency. Using the Community Needs Assessment, identifies gaps in programming and services in specific regions within York County. Develops and maintains strong working relationships with community partners and with other CPC programs to integrate and coordinate services to clients.

Requirements
ESSENTIAL DUTIES AND RESPONSIBILITIES

- Demonstrate commitment to the vision, mission, and values of Community Progress Council.
- Develop and maintain relationships with community agencies and groups to coordinate referrals and services to clients.
- Develop and maintain knowledge of community strengths and challenges to assess service implementation and/or programming needs.
- Co-facilitate and coordinate group learning in Getting Ahead in a Just Getting by World conversations.
- Provide a coaching experience using Family Centered Coaching concepts that are focused on building strong relationships to support participants in moving toward self-sufficiency.
- Interview and complete a holistic assessment using the Self-Sufficiency Scale that will help determine the client’s strengths and identify barriers; assist client in identifying their current and needed resources to achieve self-sufficiency.
- Provide individualized, intensive coaching support using a relationship-based and client centered approach.
- Coach client in identifying action steps necessary to achieve their goals, monitor progress.
- Provide support necessary to foster success through decision making, planning, budgeting, education, and financial support to allow the client to attain education and employment goals.
- Provide a coaching experience that is focused and motivating to clients and a location that is convenient to the client as needed.
- Maintain accurate client files and records of work activities.
- Serve as a mentor, advocate, or liaison on behalf of the client to ensure timely, effective, and appropriate delivery and eligibility of services while maintaining client confidentiality.
- Provide timely and accurate reports to the supervisor to meet internal and external requirements.
- Represent program/agency to community, professional, or other groups as needed.
- Attend CPC training and committee meetings.
- All other duties as assigned.

EDUCATION and EXPERIENCE: Bachelor's degree in social services or a related field, plus at least two years of related experience is preferred; OR at least 60 credits in a social service or related program of study, plus two years of related e experience. Supervisory experience is preferred.
QUALIFICATIONS: Bi-Lingual is highly preferred. OTHER REQUIREMENTS: Reliable transportation and a valid PA driver’s license. Regular and timely attendance is essential. Ability to be reached by telephone in a timely manner; ability to maintain open communication through the use of emails and work cell phones.
