About Duke HomeCare & Hospice

 

Pursue your passion for caring with Duke HomeCare & Hospice, which offers hospice, home health, and infusion services across the region, as well as serves as the home for the Duke Caregiver Support Program. Team members work closely with a patient’s physician to provide comprehensive, individualized care in the comfort of their home or at our inpatient hospice facility in Durham, NC.

The Duke HomeCare, Hospice and Home Infusion Services team is now accepting applications to the Post-Acute Transitional Care @ Home (PATCH) Program for the July 2024 cohort!

This new graduate nurse training program supports the new nurse in their first year of practice as they rotate through Home Health, Hospice, Home Infusion, and Care Coordination service lines. Nurse residents are prepared for the field of post-acute transitional care through a comprehensive training schedule while being supported by experienced preceptors, nurse educators, and managers.

 

Start Date: July 15, 2024 (RN license required before this date)
This position is seeking:  Dec 2023 to May 2024 Nursing School Graduates

 

What to Expect

• Generous comprehensive training schedule prepares new nurses with confidence as they rotate through Home Health, Hospice, Home Infusion, and Care Coordination service lines.
• Our new nurse resident will be guided and nurtured by an experienced preceptor meeting often with our nurse educator, manager, and program leaders, for additional support and encouragement.
• As a successful nurse resident, you will graduate from the PATCH program, and confidently be able to provide evidenced based care and promote positive outcomes for our patients and their loved ones.
• Successful nurse residents may choose their preferred work area in either our Home Health, Hospice, or Infusion services. Additional orientation and training will then be provided for their chosen field.

 

Duties and Responsibilities of this Level

• Plan and provide professional nursing care for patients in accordance with physician orders and established policies and procedures to include collecting and analyzing patient data, developing and implementing nursing care plans, establishing interpersonal relationships with patients, evaluating nursing care given and communicating to proper personnel relevant facts about patients, action taken and patient responses.
• Provide direction and guidance in the proper use, care and control of material resources; appraise quality and effectiveness of material and equipment and report results to head nurse; maintain standards of cleanliness in assigned areas.
• Assist in the development and implementation of general policies and procedures designed to affect efficiency of operations.
• Required Qualifications at this Level

 

Education

• BSN, Associate Degree or Diploma, Nursing required

 
Experience

• No experience required
• Preferred healthcare experience as an LPN, Medical Assistant or Nursing Assistant; community-based experience a plus.

 

Degrees, Licensure, and/or Certification

• Current registration with North Carolina State Board of Nursing as a registered nurse OR current compact RN licensure to practice in the state of North Carolina required by start date.

 

Knowledge, Skills, and Abilities

• Knowledge of scope of the registered nurse, licensed practical nurse and NCA
• Knowledge of and appropriate application of the nursing process
• Knowledge of professional theory, practice and procedure
• Ability to assess nursing needs of acute and chronically ill patients and their families
• Ability to independently seek out resources and work collaboratively
• Ability to establish and maintain effective working relationships
• Ability to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership and others
• Ability to teach patients and families in accordance with the nursing plan of care
• Ability to use sensory and cognitive functions to process and prioritize information, treatment, and follow-up
• Ability to use fine motor skills
• Competent in BLS and/or other specialized life support requirements designated by work area or organization
• Ability to record activities, document assessments, plan of care, interventions, evaluation and re-evaluation of patient status
