GENERAL DESCRIPTION

This position is part of the Hospice health team. This position is responsible for the health care needs of Hospice patients providing skilled nursing care, affording them the opportunity to remain at home and receive the medical care required.

Requirements

ESSENTIAL DUTIES

1. Completes assessment and develops plan of care appropriate to patient’s health care needs. May perform client assessments, when requested. 
2. Adheres to the established client care plan and the Physician’s Plan of Treatment. Makes ongoing additions to the client’s care plan, as needed. Follows up with executing and properly documenting any new doctor’s orders.
3. Performs assigned duties accurately and efficiently to ensure the highest patient safety, including administration of medication, treatments, and procedures per policy, monitoring the client’s condition, waived testing (glucometer and PT/INR), tracheostomy/ventilator care, and follows safety and infection control measures, etc.
4. Directs, supervises, and evaluates the performance of Home Health Aides, as applicable.
5. Maintains positive inter-personal relationships with co-workers and other members of the health care team and the organization. Exhibited by timely and appropriate communication with the physician and other health care team members regarding patient’s needs, progress, and any changes in condition.
6. Creates and supports a safe and welcoming presence for patients and their families, evidenced by client satisfaction.
7. Performs all duties, services and documentation in full compliance with CHC/SEK policies and procedures.

QUALIFICATIONS

1. Possess a valid Kansas license as a Registered Nurse or multistate license in the nurse’s state of residence.
2. One-year work experience as a Registered nurse in acute care, long-term care, outpatient services or similar field.
3. BLS Certification
4. Current Auto Insurance.
5. Valid driver’s license, as travel is required

WORK ENVIRONMENT

While performing the duties of this job, the employee is regularly required to frequently bend, stoop, squat, kneel, stand, and walk. The employee is required to frequently lift 24 pounds and push/pull 50 pounds, occasional pushing/pulling in excess of 100 pounds. Occasionally required to reach above shoulder level. Specific vision abilities required by this job include close vision, color vision, peripheral vision, depth perception and ability to adjust focus. Computer work is frequent and regular. Driving a vehicle is a daily part of this position. Environment is fast paced and occasionally stressful with tight deadlines.
