Corewell Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six month or less, serving individuals in their home, nursing facilities, and in the hospital. Summary
Under the direct supervision of the Nursing Manager, the Hospice Nurse Case Manager is responsible for coordinating and delivering compassionate end-of-life care to patients and their families. This role involves assessing patient needs, developing individualized care plans and collaborating with an interdisciplinary team to ensure the highest quality of hospice care.
Essential Functions
- Patient Assessment & Care Planning: Conduct comprehensive assessments and develop individualized care plans based on patient needs and goals of care ensuring optimal comfort and symptom management throughout the patient’s episode of care.
- Symptom Management & Pain Control: Administer medications, monitor symptoms, and adjust treatment plans to provide optimal relief.
- Interdisciplinary Collaboration: Work closely with physicians, social workers, chaplains, and other healthcare professionals to coordinate ongoing holistic care.
- Patient & Family Education: Provide support, resources, and education on disease progression, hospice philosophy, and available support services. Support patients and families in making informed decisions about care and treatment options.
- Emotional & Psychosocial Support: Provide compassionate care to patients and families, addressing emotional, spiritual, and psychological needs.
- Documentation & Compliance: Maintain accurate, compliant, and timely records of patient care, interventions, and progress while adhering to hospice regulations.
- Home & Facility Visits: Travel to patients' homes, nursing facilities, and hospitals to provide direct care.
- Professional Development: Continuously seek opportunities for learning and skill enhancement in hospice and palliative care and care management.
- Actively participates in safety initiatives and risk mitigating measures where appropriate and completes all position and unit safety related competencies and requirements on a timely basis.
- Performs other duties as assigned.
Qualifications
Required
- Associate’s degree in nursing
- 2 years of relevant experience
- Michigan Registered Nurse (RN) license upon hire
- State of Michigan Driver’s license upon hire
Preferred
- Bachelor’s degree
- 1 year of relevant experience
