We are currently seeking a RN (Clinical Lead) for our Delaware, Ohio branch. This is a part-time position, dayshift hours, and no weekends. This is perfect for a nurse looking for a more laid back role, while still being able to interact with clients and apply your experience and knowledge by training caregivers and developing plans of care. You'll work with a great team in a low stress environment.Ā Starting pay at $27.00/hr. Depending on experience.
Key Responsibilities:
- Creates and maintains the plan of care for assigned clients based on assessment of care needs, client preferences, and regular supervisory visits to client homes.Ā
- Documents all assessments, visits, and client or caregiver communication appropriately and according to Agency standards.
- Ensures compliance with documentation and plan of care standards for Passport and long-term care insurance clients.Ā
- Provides new caregiver skills training, administers HHA competency tests and skills check offs, and completes in-home training for caregivers when needed.Ā
- Performs basic nursing visits and medication management/set-up visits for private duty clients as requested/scheduled.
- Counsels the client/family on meeting daily care needs and advises client/family when to seek professional medical advice or when referral to other services is warranted.Ā
- Assesses safety issues or concerns in the home environment and determines if any action is needed to ensure the safety of the client and/or the caregivers.
- May participate in administrative after hours on-call as needed by the branch.
Knowledge/Skills:
- Excellent understanding of the current healthcare ecosystem, especially in the post-acute, long-term, and aging services industries
- Ability to effectively learn and use technology via computer, web, and mobile apps
- Excellent communication and interpersonal skills
- Proficient in the basic use of Microsoft Word, Excel, Outlook, and Teams
- Comprehensive knowledge of professional standards and principles of nursing care including state Nurse Practice Act and scope of practice for licensure
- Comprehensive knowledge of non-skilled home care scope of services and best practices
- Must be willing and able to travel from the Marion office to client homes up to 50% of the time
Requirements
- Must have a current, unrestricted nursing license in the state of Ohio, LPN or RN
- Experience working in post-acute care, long-term care, or aging services as a nurse is preferred
- Must have a valid Ohio drivers license and reliable and insured transportation
