Join our dedicated team atHospice of the Piedmont as full-time RN Case Manager in the Charlottesville and surrounding counties to manage cases assigned and provide consistent, quality patient care. Duties include but are not limited to: 

• Provide a nursing evaluation visit to new admissions
• Regularly re-evaluate patient needs
• Consult with the patient’s physician
• Complete bedside documentation using the EMR
• Coordinate and manage services offered by health team members
• Help ensure the final chapter of your patient’s life is one of quality, compassion and dignity

Requirements

Requirements

The RN Case Manager position requires graduation from an accredited nursing school and a valid license as a Registered Nurse in the Commonwealth of Virginia; two years relevant nursing experience preferred.

Typical Physical Demands/Working Conditions:

• The physical demands may require employee to regularly use hands to finger, handle or feel objects, tools, or controls, reach with hands and arms; Sit, stand, stoop, bend, reach, lift and walk, climb steps. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth vision, and ability to adjust focus
• Communication by phone, writing, speaking; Regular use of computer screen and keyboarding are required
• Lifting to 20 pounds, push up to 200 pounds in a wheelchair
• Drive short/long distances
• General fast-paced work environment
• Physical and emotional health necessary for the accomplishment of duties

• Driver’s license, personal automobile, and current automobile liability insurance
• Frequent travel may be required
